شیوع دیابت تیپ 2 پس از بارداری در زنان با سابقه دیابت بارداری : مطالعه مرور سیستماتیک و متاآنالیز

مقدمه و هدف: بیماران با سابقه ابتلا به دیابت بارداری، استعداد بیشتری برای ابتلا مجدد به این بیماری در بارداریھای بعدی دارند، ھمچنین خطر بالاتری در ابتلا به عدم تحمل گلوکز و نهایتاً ابتلا به دیابت نوع 2 در آینده دارند. ھدف از این مطالعه، تعیین شیوع دیابت و اختلال تحمل گلوکز در بیماران دیابت بارداری در دوران پس از زایمان در ایران بود.مواد و روش ها: با جستجوی اینترنتی در پایگاههای اطلاعاتی داخلی نظیر irandoc ،iranmedex ،sid ،magiran ، اسکوپوس و نیز موتور جستجوی google با استفاده از کلمات کلیدی دیابت، دیابت بارداری، بارداری، دیابت تیپ 2، دیابت حاملگی، شیوع واپیدمیولوژی google ، نسبت به جمع آوری تمام مقالات منتشر شده اقدام شد. داده های مطالعات وارد نرم افزار آماری stata شد و مورد فراتحلیل قرار گرفت. برای نشان دادن نتایج فراتحلیل از plot forest استفاده گردید.یافته ها: تعداد 6497 نمونه از مجموع 6 مطالعه در مطالعه ی حاضر بررسی شدند که در بازه سنی 18 تا 45 سال بودند. فراتحلیل شیوع دیابت پس از بارداری در6497 بیمار با سابقه دیابت بارداری 9.4٪ با فاصله اطمینان 95 درصد از 0/9 تا 0/10 بود.نتیجه گیری: اختلال متابولیسم گلوکز در دوران پس از زایمان در زنان مبتلا به دیابت بارداری اختلال شایعی باشد. بنابراین در وهله نخست غربالگری و شناسایی موارد دیابت بارداری و درمان آنها در دوران بارداری، جهت پیشگیری از عوارض جنینی و سپس پیگیری این مادران در دوران پس از زایمان، جهت شناسایی دیابت و اختلال تحمل گلوکز و پیشگیری و درمان این اختلالات میتواند حایز اهمیت باشد

Prevalence of Type II Diabetes After Pregnancy in Women with a History of Gestational Diabetes: A Systematic Review and Meta-Analysis

Introduction: Patients with a history of gestational diabetes have a greater susceptibility to recurrence in subsequent pregnancies, as well as a higher risk of developing glucose intolerance and, of course, type 2 diabetes in the future. The present study aimed to determine the prevalence of diabetes and glucose intolerance disorder in patients with gestational diabetes in the postpartum period in Iran.Materials and Methods: The he following databases were searched for all the published articles: Irandoc, Iranmedex, SID, Magiran, Scopus, as well as Google search engine using the keywords of diabetes, gestational diabetes, pregnancy, type II diabetes, pregnancy diabetes, multiple sclerosis, and epidemiology. The study data was entered the STATA statistical software and was analyzed. The forest plot was used to demonstrate the results of the metaanalysis.Results: A number of 6497 participants within the age range 1845 years from 6 studies were assessed in the current study. According to the results of the metaanalysis, the prevalence rate of postpartum diabetes in 6497 patients with a history of gestational diabetes (9.4%) and a confidence interval of 95% was obtained at 9.0 10.0.Conclusion: Postpartum Glucose Metabolism Disorders are common in women with gestational diabetes. Therefore, the cases of gestational diabetes should be initially screened, identified, and treated during pregnancy to prevent fetal complications. Thereafter, these mothers need to be followed up in the postpartum period to identify diabetes and glucose intolerance and prevent and treat these disorders.
