پرستاری از راه دور در مراقبت بیماری‌های مزمن: یک مرور نظام‌مند

مقدمه: بیماریهای مزمن دارای دوره طولانیمدتی بوده و به طور آهسته پیشرفت میکنند و هزینه های بسیار زیادی را به بیماران و دستگاههای ارائه دهنده مراقبت های بهداشتی کشورها تحمیل می نمایند. این بیماران به مراقبت، آموزش و راهنمایی های مداوم و طولانی مدت گروه درمان نیازمند هستند. پرستاری از راه دور توانایی ارائه این خدمات را به بیماران با کمترین هزینه و بیشترین دسترسی فراهم میآورد. هدف از این مطالعه بررسی به کارگیری پرستاری از راه دور در انواع بیماریهای مزمن و نتایج حاصل از آن می باشد. روش: در این مطالعه مروری نظام مند تمامی مقالات فارسی و انگلیسی تمام متن در زمینه پرستاری از راه دور در بیماریهای مزمن با کلیدواژه های فارسی پرستاری از راه دور، پی گیری تلفنی، بیماری های مزمن، پرستاران و هزینه اثربخشی موردبررسی قرار گرفتند. گستره جستجوی مقالات فارسی بانکهای اطلاعاتی sid، magiran، iranmedex، nindex و سایت وزارت بهداشت از تیرماه 1383 لغایت تیرماه 1393 بود. در مورد مقالات انگلیسی زبان بانکهای اطلاعاتی بین المللی scopus, pubmed, proquest, elsevier, ovid با کلیدواژه tele nursing, health, cost benefit, chronic patients effectiveness نیز در بازده زمانی 10 ساله از اوت 2004 لغایت اوت 2014 موردبررسی قرار گرفتند و درنهایت 31 مقاله وارد مطالعه گردید. یافته ها: از میان 31 مقاله موردبررسی 8 مقاله در مجلات داخلی و 23 مقاله در مجلات خارجی منتشرشده بودند. 26 مقاله (84 درصد) در مورد کاربرد پرستاری از راه دور و نتایج حاصل از آن در بیماریهای مزمن نارسایی قلبی، دیابت نوع 2 و بیماریهای مزمن انسدادی ریوی بودند. نتیجه گیری: پرستاری از راه دور در بیماران مزمن منجر به افزایش کفایت در رعایت رژیمدرمانی، کاهش مرگ ومیر، کاهش هزینه ها، کاهش افسردگی و اضطراب، کاهش دفعات بستری و افزایش کیفیت زندگی در این بیماران میگردد.

Tele nursing care in chronic patients/a systematic review

Introduction: Chronic diseases as defined have long standing periods and slowly progress. These conditions are very costly for patients and countries health care systems. The Patients need lon>erm care and ongoing training and guidance about the treatment from the team. Tele nursing has the capacity to facilitate the access of the patients to these services at the lowest cost. The purpose of this study was to evaluate some common types of chronic disease in which tele nursing has been used and the outcomes of this type of care is reported. Methods: In this systematic review, the full texts of all articles in Persian and English related to reports of tele nursing in chronic diseases searched by these keywords: Tele nursing, followup with phone calls, chronic illness, nurses and cost effectiveness. Persian medical and allied health care databases included Magiran, Iran, Medex, SID, Nindex, Ministry of Health website and Englishlanguage articles on following international databases: ProQuest, PubMed, ELSEVIER, Ovid,Scopus retrieval of articles spanned From August 2004 until August 2014. Results: Of the 31 articles reviewed 8 articles were in Iranian journals and 23 articles were published in international journals. Focus of the 26 articles (84%) were on the use of remote nursing care and outcomes in chronic heart failure, type 2 diabetes and chronic obstructive pulmonary diseases. Conclusion: Providing tele nursing cares in chronic patients have led to increased efficiency in adherence to the treatment regimen, reduced mortality rates and costs, reduced depression and anxiety, decreased hospitalizations and improvement of quality of life in patients.
