نیازهای توانبخشی بیماران با ناتوانی جسمی ناشی از حوادث ترافیکی برای بازگشت به جامعه: یک مطالعه کیفی

مقدمه:افزایش روزافزون حوادت و سوانح ترافیکی منجر به افزایش مصدومین و معلولین گردیده که بدرجات مختلفی دارای انواع محدودیتها و معلولیت های جسمی هستند، خوشبخنانه پیشرفتهای روز افزون علوم پزشکی، پرستاری و توانبخشی منجر گردیده بیشتر افراد با ناتوانیهای جسمی، توان بازگشت به جامعه را بدست آورند، برای بازگشت به جامعه باید نیازهای افراد ناتوان بخوبی شناخته شود تا با رفع آنها زمینه حضور در جامعه برای آنها فراهم گردد. هدف از این مقاله بررسی و درک نیازهای توانبخشی بیماران با ناتوانی جسمی ناشی از حوادث ترافیکی برای بازگشت به جامعه است. روش پژوهش: این مطالعه با رویکرد تحقیق کیفی و با روش تحلیل محتوای کیفی انجام گردید، 12 مشارکت کننده از هر دو جنس با دامنه سنی 18 تا 45 ساله با سابقه حداقل 3 ماه ناتوانی جسمی اندام فوقانی ، تحتانی,و یا صدمات طناب نخاعی با روش نمونه گیری هدفمند از دو مرکز تخصصی بیمارستانی و کلینیک های وابسته به دانشگاه علوم پزشکی تهران و ایران انتخاب شدند. داده ها از طریق مصاحبه های نیمه ساختارمند جمع آوری و تا اشباع داده ها ادامه یافت. تجزیه و تحلیل داده ها به طور مستمر و همزمان با جمع آوری داده ها و به صورت مقایسه ایی با رویکرد تجزیه و تحلیل محتوای قراردادی انجام گردید. یافته ها: بر اساس یافته های این مطالعه سه طبقه اصلی این مطالعه شامل نیازبه قرار گیری درچتر حمایتی، نیاز به تداوم مراقبتی، و نیل به استقلال مشخص گردید. در حقیقت نیازهای این مددجویان مورد حمایت قرار گرفتن از سوی دیگران شامل خانواده و جامعه، استمرار مراقبتها در سراسر دوران معلولیت و نیاز به استقلال بوده است. نتیجه گیری: نیازهای شناخته شده در این تحقیق باید در طراحی برنامه های مراقبتی مددجویان گنجانده شود. لذا پیشنهاد می گردد، برای دستیابی ناتوانان جسمی ناشی از سوانح ترافیکی به استقلال، فراهم نمودن حمایت کافی آنها و همچنین در دسترس قراردادن و تداوم مراقبتها برای آنها، از رویکرد توانبخشی خانواده و جامعه محور با مدل توانبخشی چند بعدی فراگیر که می تواند هر سه دسته نیاز فوق تامین نموده و پوشش دهد، استفاده گردد.

Rehabilitation needs of patients with physical disabilities due to traffic accidents for returning to the community: a qualitative study

Introduction: Increase in road traffic accidents has led to increase in various degrees of injuries and disabilities with various physical limitations and disabilities. Fortunately, increasingly advances in sophisticated medical, nursing and rehabilitation sciences has led to most people with physical disabilities, would have be enable to return to the society. In order to return to the community the needs of people with disabilities should be well known as well as their presence in the community to be provided. The purpose o f this study was to understand the rehabilitation needs of patients with physical disabilities from road traffic accidents to return to the community. Methods: A qualitative approach using conventional content analysis was adopted. 12 participants of both genders and age ranged between 1845 years old with at least 3months physical disability in upper and lower limbs or spinal cord injury of two specialized hospitals and clinics affiliated to Tehran and Iran Universities of Medical Sciences were selected through purposive sampling. Data collected through indepth semistructured interviews and continued until data saturation. Data analysis was done continuously and simultaneously with data collection. Also constant comparison method with conventional content analysis approach was used. Findings: Three themes emerged from the data analysis, including: (a) the need to be under the umbrella of support, (b) the need for continuity of care and (c) achieving independence. In fact, the needs of the clients were getting support from others, including family and community, continuity of care throughout their disability and the need for independence. Conclusion: The needs identified in this study should be included in the designing of client care plans. Therefore, to achieve the independence of the physically disabled due to traffic accidents, providing sufficient support as well as the availability and continuity of care is suggested. Also a multidimensional rehabilitation model with the family and communitybased rehabilitation approach should be used to provide the necessary cover to these three categories.
