تاثیر الگوی توانمندسازی خانواده محور برخودکارآمدی بیماران تحت همودیالیز

مقدمه: نارسایی مزمن کلیه اختلال پیشرفته و برگشت ناپذیر عملکرد کلیه می باشد که امروزه 2تا3درصد مردم دنیا به آن مبتلا می باشند. براساس امار ارائه شده از سوی مرکز پیوند بیماری های خاص تعداد بیمارانی که دچار نارسایی پیوند کلیه هستند حدود25000نفر می باشد . سالانه حدود4000بیمار جدید به این تعداد اضافه می گردد در این بین دیالیز به عنوان رایجترین روش درمانی در این بیماران به شمار می آید، تقریبا بیش از 50درصد این بیماران کلیوی در ایران تحت درمان با همودیالیز می باشند. بیماران همودیالیزی به دلیل ماهیت مزمن بیماری دچار عوارض و مشکلات زیادی هستند که می تواند موجب کاهش خودکارآمدی و کیفیت زندگی آنان گردد. بهبود و حمایت از این بیماران نیازمند مشارکت فعال خانواده می باشد. بنابراین این مطالعه با هدف تاثیرالگوی توانمندسازی خانواده محور برخودکارآمدی بیماران تحت همودیالیز استان گلستان انجام گرفت.روش کار: این مطالعه کارازمایی بالینی برروی70 نفر از بیماران همودیالیزی شهرگنبدکاووس که بعد از همسان سازی از نظر تعداد دفعات دیالیز، تحصیلات، سن و جنس به صورت تصادفی ساده به دو گروه آزمون و کنترل تقسیم شدند. الگوی توانمندسازی خانواده محور در گروه آزمون در6جلسه 9060دقیقه ای اجرا گردید. ابزار گردآوری اطلاعات پرسشنامه خودکارآمدی عمومی شرر بود. تجزیه تحلیل اطلاعات توسط نرم افزارآماری spss21با آزمون آماری تی مستقل، تی زوجی و آنالیزواریانس صورت گرفت.یافته ها:آزمون تی مستقل بین گروه آزمون و کنترل قبل از مداخله اختلاف معنی داری را نشان نداد. (0/97= p).ولی تی مستقل بعد از مداخله بین دو گروه اختلاف معنی داری را نشان داد (p<0.01)نتیجه گیری: اجرای الگوی توانمندسازی خانواده محور با مشارکت اعضای خانواده موجب افزایش خودمراقبتی و ارتقاء سبک زندگی بیماران همودیالیزی می گردد. لذا با توجه به کم هزینه و ایمن بودن و تاثیر گذاری الگوی توانمندسازی خانواده محور پیشنهاد می گردد از این الگو بر روی سایر متغیرهای مراقبت پرستاری و یا سایر بیماران مزمن استفاده گردد.

The Impact of Family-Based Empowerment Pattern on Hemodialysis Patients in Golestan Province in 1394

Introduction: Chronic renal failure is an advanced and irreversible impairment of renal function, which affects 2 to 3 percent of the world's population today. The number of patients with kidney transplant failure is estimated to be about 25,000, according to a report from the Center for the Transplantation of Specific Diseases. Annually, about 4,000 new patients are added to this population, with dialysis as the most commonly used treatment. Nearly 50% of these kidney patients are hemodialysis patients in Iran.Hemodialysis patients have many complications due to the chronic nature of the disease, which can decrease their selfefficacy and quality of life. Improving and supporting these patients requires active participation of the family. Therefore, the aim of this study was to improve the familycentered empowerment of hemodialysis patients in Golestan province.Methods: This experimental study was carried out on 70 hemodialysis patients in GonbadeKavoos city who were randomly divided into two groups of control and control after being matched for dialysis, education, age and sex. The familycentered empowerment model in the test group was performed in 6 sessions of 6090 minutes. Data gathering tool was Sherer general selfefficacy questionnaire. Data analysis was done by SPSS21 software using independent ttest, paired Ttest and Analysis of variance.Results: Independent ttest between the intervention and control groups did not show any significant difference. (P = 0.97), but independent t after intervention showed significant difference between the two groups (P < 0.01).Conclusions: Implementation of familycentered empowerment model with the participation of family members will increase selfcare and promote lifestyle in hemodialysis patients. It is recommended that the patient receive the health care in the family home. Due to the low cost and safety and the impact of the familycentered empowerment model, it is recommended to use this model on other variables of nursing care or other chronic patients.
