بررسی موانع ارائه مراقبت پایان زندگی به کودکان در حال مرگ از دیدگاه پرسنل پرستاری

مقدمه: توسعه فن آوری و درمانهای دارویی موجب زنده نگهداشتن طولانی مدت کودکان مبتلا به بیماریهای مزمن و افزایش نیازهای مراقبت پایان زندگی آنان شده است. کودکان در حال احتضار و خانواده آنان نیازمند مراقبت جامع پایان زندگی هستند. پرستاران در ارائه اینگونه مراقبت ها با چالشهای زیادی مواجه می شوند. مطالعه حاضر با هدف بررسی موانع ارائه مراقبت پایان زندگی کودکان در حال مرگ, در بخشهای کودکان بیمارستانهای شهر کرمان انجام شد.روش: در یک مطالعه توصیفی تحلیلی 151 نفر از پرسنل پرستاری به روش نمونه گیری در دسترس انتخاب شدند . داده ها با استفاده از پرسشنامه های مشخصات جمعیت شناختی و پرسشنامه ی (nsccnreol)( شامل 29گویه که شدت و تکرر موانع بر اساس مقیاس 4 درجه ای لیکرت به تر تیب ((بیشترین مانع =3، مانع نیست =0) و (همیشه=3 ، هرگز=0)) جمع آوری شدند.یافته ها: مانعِ عدم پذیرش مرگ احتمالی کودک توسط خانواده بالاترین میانگین نمره (5/40) ؛ و مانعِ ادامه درمان های پیشرفته کودک به دلیل وجود منافع مادی برای بیمارستان پایین ترین میانگین نمره (2/09) را کسب کردند. بین داشتن تجربه مرگ افراد فامیل یا دوستان نزدیک ( p=0/02) و سابقه کار پرستاری (p=0/02 ) با اهمیت موانع, ارتباط معنی داری وجود داشت.نتیجه گیری: بر اساس یافته های این پژوهش عدم آموزشهای مناسب مراقبت پایان زندگی و وجود بعضی از محدودیتهای فرهنگی و حرفه ای منجر به نقایصی در زمینه ارائه مراقبت پایان زندگی مطلوب گردیده است. تقویت آموزشهای مراقبت پایان زندگی و تسکینی بر اساس اعتقادات اسلامی و فرهنگ ایرانی سبب افزایش دانش و مهارت پرستاران در رویارویی با چالشهای مراقبت از بیماران در حال مرگ خواهد شد.

Assessment of Barriers in providing end of life care to terminally ill pediatric patients from the perspective of nursing staff

Introduction: Development in technology and pharmaceutical treatments have increased the number of children with life threatening conditions as well as their end of life care requirements. These children and their families require comprehensive and appropriate end of life care. Nurses are faced with challeng in their efforts to provide end of life care. This study was aimed to assessment of Barriers in providing end of life care to terminally ill pediatric patients in kerman hospital. Methods: In a descriptive analytic study,151 nursing staff were Selected with Purposive sampling .A translated modified version of (NSCCNREOL) and demoghraphic questionnaire was used to data collection. Results: The highest perceived barrier magnitude score was families not accepting poor child prognosis rdquo; (5/40) and the lowest perceived barrier magnitude score belonged to the item ldquo;Continuing to provide advanced treatment to dying child because of financial benefits to the hospital rdquo; (2/09). Between Participants rsquo; family and closed friends rsquo; death experience positively correlated with perceived organizationalrelated barriers magnitude (p=0/02). Also participants rsquo; nursing experience positively correlated with perceived healthcare professional related barriers magnitude (p=0/02).Conclusion: The study findings showed that lack of education and experience as well as some cultural and professional limitations may have contributed to some deficiencies in providing end of life care. So developing end of life and Palliative care education may enhance nurses rsquo; knowledge and skill to face the challenges of end of life care. Establishing Palliative care units in the community as well as developing of palliative home care programs that focus on family members, who play an important role in children rsquo;s care may have positive effect on pediatric end of life care.
