   The effect of 0.9% NaCl or ringer's lactate infusions on metabolic acidosis in tourniquet applicated total knee arthroplasty under spinal anesthesia [Spinal anestezi Altinda Turnike Uygulanmiş Total Diz Artroplastisinde %0.9 NaCl veya Ringer Laktat İnfüzyonlarinin Metabolik Asidoza Etkisi]

چکیده    Objective: the aim of this study is to compare the effects of 0.9% nacl and ringer's lactate solutions in acid-base balance in patients under spinal anesthesia undergoing total knee arthroplasty (tka) with tourniquet application associated with inflation and deflation of the tourniquet. method: asa i-ii60 patients age>40,undergoing tka with tourniquet application under spinal anesthesia were included in the study. spinal anesthesia was performed after iv infusion of 10 ml kg1 0.9% naci to group 1 and 10 ml kg1 ringer's lactate to group 2. in maintenance 10 ml kg1 h1 infusion was given to each groups. in all patients' base (to),5th min after spinal anesthesia (tl),15th min after tourniquet inflation (t2),1st min (t3),15th min (t4),30th min after tourniquet deflation,mean arterial blood pressure and heart rate were recorded. moreover,in to,t2,t3,t4,t5 measurement time arterial and venous blood gase ph,po2,pco2,lactate,hco3,chlorine (cl) levels were recorded. results: in group one,ph values were significantly lower and chlorine values were significiantly higher from group two. there was no significant difference between the groups,in heart rate,blood pressure and lactate levels. conclusion: in patients undergoing total knee arthroplasty under spinal anesthesia with tourniquet application,infusion of 0.9% nacl solution caused hyperchloraemic metabolic acidosis. it's shown that metabolic acidosis doesn't occur in the ringer's lactate group.
