Effects of endotracheal intubation or proseal laryngeal mask insertion on hemodynamics,ventilation,and gastric distension in laparoscopic cholecystectomy [Laparoskopi̇k kolesi̇stektomi̇de endotrakeal entübasyon veya proseal lari̇ngeal maske uygulamasinin hemodi̇nami̇,venti̇lasyon ve gastri̇k di̇stansi̇yon üzeri̇ne etki̇leri̇]

Objective: the effects of endotracheal intubation (ett) or proseal laryngeal mask (plma) insertion on hemodynamics,ventilation,and gastric distension in laparoscopic cholecystectomy was evaluated. method: eighty patients,asa i-iii scheduled for laparoscopic cholecystectomy were randomly divided into two groups. endotracheal intubation was performed for ett group,and plma was inserted for plma group. standard types of anesthesia and surgery procedures were applied for all patients. heart rate (hr) and mean arterial pressure (map) were recorded before application of the airway; 1,3,and 5th minutes after application; and 1,3,and 5th minutes after extubation. peripheral o2 saturation (spo2),minute ventilation (vmin),peak airway pressure,endtidal co2 (etco2) values were recorded two times; before the insufflation and 15 minutes after insufflation. gastric distension was determined visually by the same surgeon after insufflation and 5 minutes before exsufflation and recorded with a score between 0 and 10. results: hr and map values after application of the airway in first and third minutes were significantly found higher in ett group compared to plma group (p< 0.05). the peak airway pressure and etco2 values which were recorded at 15 minutes after insufflation were found significantly higher (p< 0.05) than the values recorded before insufflation within groups in both groups. but,there were no significant differences between groups. the groups were found similar when compared for spo2,vmin,and gastric distension. conclusion: we concluded that the application of plma is a reliable alternative method to endotracheal intubation in laparoscopic surgery in which increased intra-abdominal pressure may result in restrictions on the parameters of ventilation.
