The effects of different types of anesthesia on fetal IL-6 levels and natural killer cell activity [Farkli anestezi̇ yöntemleri̇ni̇n fetal IL-6 düzeyleri̇ ve natural killer (NK) hücre akti̇vi̇tesi̇ üzeri̇ne etki̇leri̇]

Objective: that they have insufficient immunity,mothers' immune system have an modulator impact to fetus; obstetrics anesthesia methods used is important. we investigated that the difference between different types of anesthesia on fetal il-6 levels and nk cell activity in this study. method: thirty patients undergoing elective caesarean section were assigned to two groups: in spinal anesthesia group (n:15),anesthesia was performed with hyperbaric bupivacaine and fentanyl. in general anesthesia group (n:15),induction was performed with thiopenthal,succinylcholine and sevoflurane. after clamping the umbilical cord,fentanyl and vecuronium were administered. maternal blood was collected for il-6 levels at preoperative period and final closure of the skin incision. after neonatal delivery,cord blood was collected for nk cell activity,il-6 and cortisol levels. results: the time interval from the beginning of anaesthesia to umblical cord clamping (abzkkz) was longer in spinal anaesthesia group (p<0.05) and there was negative correlation between this time and fetal il-6 levels (p<0.05). pre- postoperative maternal and fetal il-6 levels were not influenced by anesthesia techniques (p>0.05). fetal cortisol levels were higher in general anesthesia group (p<0.05). there was no difference between pre-and postoperative maternal il-6 levels within groups (p>0.05). nk cell activity was smilar in both groups (p>0.05). there was negative correlation between nk cell activity and fetal il-6 levels in general anesthesia group (p<0.05). conclusion: longer abzkkz and its negatively correlation with fetal il-6 level in spinal anesthesia group and higher fetal cortisol level in general anesthesia group may be important for fetal defense to stress and infection.
