Comparison of spinal anaesthesia and paravertebral block for inguinal hernia operations [İngui̇nal herni̇ operasyonlarinda spi̇nal anestezi̇ i̇le paravertebral blok uygulamalarinin karşilaştirilmasi]

Objective: the aim of the study is to compare for inguinal hernia repair operated under paravertebral nerve block (pvb) or spinal anesthesia (sa). method: forty asa i-ii patients,aged 18-65 years,scheduled for unilateral inguinal hernia repair were randomly assigned into two groups. twenty asa i-ii patients in group s (n=20) received 3 ml 0.5% hyperbaric bupivacaine intrathecally,and 20 asa i-ii patients in group p received pvb at t10-t11 levels with 5 ml 0.5% plain bupivacaine results: block application duration time (16.2±4.0 vs 6.2±2.1 minute),time to adequate sensory block (20.8±5.6 vs 6.4±1.9 hour),time until first pain after operation (13.0±3.4 vs 3.0±1.0 hours) was found to longer in group p with respect to group s (p< 0.001). mean time to first mobilization (421.0±91.4 vs 1813±51.8 minute),time to achieve discharge criteria (12.7±5.8 vs 4.4±1.2 hours) was longer in group s with respect to group p (p< 0.001). in group p all patients received propofol sedation (905±20.6mg),there were no patients received propofol sedation in group s. postoperative 24 hours tramadol consumption (248.0±39.4 vs 363.0±61.6mg),postoperative visual analogue scale (vas) at 6. hour (2.4±1.9 vs 5.6±1.7),12. hour (2.6±1.1 vs 5.4±1.4) was lower in group p (p< 0.001). because of pain,we used general anesthesia in one patient (5%) in group p. except decline in mean arterial pressure after block in goup s (p< 0.001); hemodynamic changes and patient satisfaction between groups were similar (p≥ 0.05). conclusion: when compared with sa,pvb provided longer postoperative analgesia and less analgesic consumption,the time to achieve discharge criteria was shorter and the number of patients having side effects were lower in pvb group. nevertheless the time the procedure takes to perform in pvb group was longer than sa group.
