The comparison of the effects of propofol-ketamine and midazolam-ketamine in patients undergoing colonoscopy [Kolonoskopi̇ yapilan hastalarda propofol-ketaṁin ve ̇midazolam-keṫȧminiṅ eṫk̇ileri̧niņ karsilastirilmasi]

Objective: colonoscopy is an endoscopic procedure for diagnosis,screening and treatment. it is uncomfortable and painful,so sedation is preferred. we aimed to compare the efficacy and adverse effects of midazolam plus ketamine and propofol plus ketamine during colonoscopy. method: after the ethics committee approval,100 patients asa 1-11,age 18-70 were enrolled. state trait inventory (stai) was used to measure the anxiety levels before colonoscopy. the mk group (n=50) recieved 0,05 mg kg-1 midazolam and 0,5 mg kg-1 of ketamine,the pk group (n=50) recieved 1 mg kg-1 propofol and 0,5 mg kg-1 of ketamine intravenously. depth of sedation was evaluated by observer assessment of alertness and sedation scale. recovery,discharge times,additional medication and side effects were recorded. results: no difference was observed regarding demographic data between the groups. state anxiety inventory for women (50 ± 10),were higher than men (42 ± 9) (p<0.05). heart rate values were significantly lower than the baseline at 9th,12th,15th,20th,25th and 30th minutes for group mk and at 6th,9th and 12th minutes for group pk (p<0.05). map values one minute after induction were lower than the baseline in both groups (p<0.05). the times to cecal intubation,anesthesia and discharge were significantly longer in the mk group (p=0.038,p=0.010,p=0.011). adverse effects,need for additional medication and oxygen did not differ between the groups. conclusions: propofol-ketamine and midazolam-ketamine combinations for colonoscopy,produced similar hemodynamic and sedation conditions,but discharge was comparatively shorter with propofol-ketamine. therefore,propofol-ketamine for the sedation of colonoscopy might be beneficial.
