چکیده    Elderly patients differ from younger patients in terms of physiological,pharmacological,psychological and social characteristics. it is known that complications related to anesthesia in the geriatric patients are higher compared to the young people. in particular,anesthesia applied to the patients with advanced age (over 80 years) is highly important. cognitive deterioration (delirium,confusion,etc.) is an important problem in early postoperative period in the elderly patients. it has been reported that general anesthesia in elderly patients may lead to cognitive disorder in early postoperative period while regional anesthesia leads to fewer complications compared to general anesthesia. a transurethral resection of the prostate was carried out under spinal anesthesia twice with 75 days interval successfully in a 102-year-old patient with inability to urinate,dysuria and urinary incontinence due to prostate malignancy. he was previously diagnosed with diabetes mellitus type ii and hypertension. his preoperative laboratory values were in normal range,body mass index was 23.4 and american society of anesthesiologists physical status (asa) was asa-iii. in conclusion,we believe that after evaluating each elderly patient individually in terms of negative effects of anesthesia,spinal anesthesia with a good preparation and careful management may be more favorable compared to general anesthesia.
