مدیریت تعارض خط‌مشی در فرآیند خط‌مشی‌گذاری نظام سلامت ایران؛ عناصر، آثار و راهکارها

هدف پژوهش حاضر، ارائه عناصر موثر بر شکل‌گیری، آثار بالقوه و راهکارهای مدیریت تعارض خط مشی است. برای پاسخ به سوال اصلی پژوهش که عبارت است از «عناصر موثر بر مدیریت تعارض خط‌ مشی در فرآیند خط‌ مشی‌گذاری نظام سلامت ایران چیست؟» از رویکردی کیفی و روش‌های دلفی و تحلیل مضمون استفاده شد. برای اجرای روش دلفی، نظر 13 نفر از خبرگان در قالب سه دور پرسش نامه دریافت شد. شبکه مضامین مدیریت تعارض خط مشی های نظام سلامت با 111 مضمون پایه، 14 مضمون سازمان دهنده و 3 مضمون فراگیر شناسایی شد. یافته‌های پژوهش در خصوص عناصر موثر بر شکل‌گیری تعارض شامل نوع بازیگران، خصوصیات ایدئولوژیک نقش آفرینان، بسترهای اجرا، ویژگی های محیطی و طراحی عناصر درونی خط مشی استخراج شد. همچنین مضامین حاصل از آثار بالقوه تعارض شامل آثار اجتماعی، فرآیندی، حاکمیتی و انعکاسی شناسایی گردید. تصمیمات مبتنی بر شواهد، توسعه مشارکت پذیری، تعمیق شفافیت، تنظیم گری قانونی و مدیریت تعارض منافع نیز به عنوان راهکارهای مدیریت تعارض خط‌مشی مشخص شد.

Policy Conflict Management in the Health System Policymaking Process Elements, Effects and Solutions

The purpose of this study is to present the elements affecting the formation, potential effects and strategies of policy conflict management. To answer the main research question, which is the elements affecting the management of policy conflict in the policymaking process of the Iranian health system? A qualitative approach and Delphi methods and content analysis were used. To implement the Delphi method, the opinions of 13 experts were received in the form of three rounds of questionnaires. The network of general policy conflict management themes was identified with 111 basic themes, 14 organizing themes and 3 comprehensive themes. The research findings were extracted from the Elements of conflict formation including the type of actors, ideological characteristics of the rolemakers, performance contexts, environmental characteristics and design of internal elements of the policy. Findings from the effects of conflict including social, process, governance and reflection effects were also identified. Evidencebased decision making, participatory development, deepening transparency, legal regulation and conflict of interest management were also identified as policy conflict management solutions.
