اهمیت رویکرد فراتشخیصی در آسیب شناسی و روان درمانی

در سال های اخیر، استفاده از اصطلاح و سازه »فراتشخیص « چه در حوزه آسیب شناسی و چه در حوزه درمان های روان شناختی مورد توجه بوده است. مدل ها و درمان های فراتشخیصی در نقطه مقابل درمان های مبتنی بر پروتکل های تک تشخیصی به وجود آمدند. فراتشخیص در حوزه آسیب شناسی، به معنی پرداختن به سازوکارهای زیربنایی مشترک در طیفی از اختلال های هم ابتلا و در حوزه درمان، به معنی داشتن پروتکل های درمانی است که صرفاً بر اساس تشخیص های اختصاصی مداخله نمی کنند و این سازوکارهای زیربنایی مشترک را در درمان مدنظر قرار می دهند. هدف از مقاله حاضر، تبیین معانی مختلف سازه فراتشخیصی و ارائه مختصری از دو مدل و درمان مطرح در این حوزه، یعنی درمان پروتکل واحد و درمان تنظیم هیجان است. در انتها با توجه به چالش های مربوط به هم ابتلایی اختلال های روانپزشکی و سهولت اشاعه و درمان در وضعیت فراتشخیصی، به مزایا و معایب این دیدگاه پرداخته می شود.

The importance of transdiagnostic approach in psychopathology and psychotherapy

In recent years, the use of the ldquo;transdiagnostic rdquo; term and construct has been considered in the field of psychopathology and psychological treatments. Transdiagnostic models and treatments were developed in contrast to singlediagnosis protocols. Transdiagnostic in psychopathology means addressing common underlying mechanisms in a range of comorbid disorders and in the field of treatment means we provide treatment protocols that do not merely intervene based on specific diagnoses and treat these underlying mechanisms. In this regard, the present article aims to explain the different meanings of transdiagnostic construct and to present a brief overview of two proposed transdiagnostic models and treatments; unified therapy and emotion regulation therapy. Finally, considering the challenges of comorbidity associated with psychiatric disorders and the ease of dissemination and treatment in transdiagnostic status, the advantages and disadvantages of this approach are discussed.
