بررسی جامعه شناختی مطالبات پزشکی مصدومین جراحی زیبایی در کمیسیون های پزشکی قانونی استان تهران

سابقه و هدف: با افزایش اعمال جراحی زیبایی، قصورات پزشکی نیز متعاقبا افزایش می یابد. این مطالعه با هدف بررسی مطالبات پزشکی (تعهد پزشک و رضایت از پزشک) افراد مصدوم و آسیب دیده بدنبال جراحی زیبایی از پزشک قانونی می باشد.روش کار: در این مطالعه مقطعی توصیفی-تحلیلی، 276 نفر از شاکیانی که در سال های 1398 و 1399 قصور پزشکی آنها در کمیسیون های پزشکی قانونی استان تهران دو بار مورد تایید قرار گرفته بود وارد مطالعه شدند. این مطالعه پس از تایید در دانشگاه آزاد اسلامی، سپس در کمسیون های پزشکی قانونی استان تهران نیز مورد تایید قرار گرفت (ir.iau.srb.rec.1400.290). پرسشنامه محقق ساخته جهت بررسی تعهد پزشک و رضایت از پزشک شرکت کنندگان استفاده شد. از نرم افزار spss ورژن 22 برای تحلیل داده ها استفاده گردید.یافته ها: میانگین سنی بیماران 37 سال بود. 18 نفر (6.5 درصد) از پاسخگویان مرد و 258 نفر (93.5 درصد) از پاسخگویان زن بودند. از نظر دفعات جراحی،49.6 درصد از شاکی ها یکبار و 31.9 درصد از پاسخگویان دوبار جراحی زیبایی کردند و میانگین دفعات جراحی در میان شاکیان تقریبا 2 و بیشترین دفعات جراحی 8 بار است. میزان شکایت در بین زنان و پاسخگویان متاهل و با تحصیلات بالا بیشتر از بقیه گروه ها بوده است. اکثر پاسخگویان بیشتر جراحی لیپوساکشن (8%/18) و رینوپلاستی (1%/18) داشتند. نتایج پژوهش حاکی از آن است که بین میزان شکایت از جراحی زیبایی و مطالبات پزشکی؛ رضایت بیمار (0.528-=r) و تعهد پزشک (0.751-=r) رابطه معکوس و معناداری وجود دارد. در واقع با کاهش رضایت بیمار و تعهد پزشک میزان شکایت از جراحی نیز افزایش می یابد.نتیجه گیری: طبق نتایج مطالعه، دو مولفه مطالبات پزشکی شامل تعهد پزشک و رضایت بیمار پیش گو کننده مهمی برای شکایات از جراحی زیبایی می باشند و تعهد پزشک بیشترین تاثیر را بر میزان شکایت از جراحی زیبایی دارد. با افزایش هر مولفه از مطالبات پزشکی، احتمال شکایت بدنبال جراحی زیبایی کاهش پیدا می کند و بالعکس.

sociological study of medical demands of cosmetic surgery victims in forensic medical commissions of tehran province

background and aim: medical malpractice increases with the increase of cosmetic surgeries. this study aimed to investigate the medical demands (physician's commitment and patient satisfaction) of injured people seeking cosmetic surgery from a forensic medicine commission. methods: in this descriptive-analytical cross-sectional study, 276 subjects whose medical malpractice was confirmed twice by the forensic medical commissions of tehran province in 2019 and 2020 were included. after being approved by islamic azad university, this study was also approved by forensic medicine commissions of tehran province (ir.iau.sab.rec.1400.290). a researcher-made questionnaire was used to check the physician's commitment and patient satisfaction of the participants. spss version 22 software was used for data analysis. results: the mean age of the patients was 37 years. regarding gender, 18 respondents (6.5%) were male and 258 respondents (93.5%) were female. regarding the frequency of surgery, 49.6% of the respondents had cosmetic surgery once and 31.9% had cosmetic surgery twice, and the mean number of surgeries was 2 times and the maximum was 8 times. the number of complaints among married women and respondents with high education was higher than other groups. most respondents had liposuction surgery (18.8%) and rhinoplasty (18.1%). the results of the research indicated that between the amount of complaints about cosmetic surgery and medical demands; patient satisfaction (r=0.528) and physician commitment (r=0.751) have an inverse and significant relationship. in fact, with the decrease in patient satisfaction and physician commitment, the amount of complaints about cosmetic surgery also increases. conclusion: according to the results of the study, two components of medical demands, including physician's commitment and patient satisfaction, are important predictors for cosmetic surgery complaints. physician's commitment has the greatest impact on the amount of cosmetic surgery complaints. as each component of medical demands increases, the likelihood of a complaint after cosmetic surgery decreases and vice versa.
