از ثریا تا ثریا: خود تعالی در یک مورد بهبودیافته پیوند سلول‌‌های بنیادی خون‌ساز

مقدمه: پیوند سلول‌های بنیادی خون‌ساز یک درمان بالقوه، همچنین یک روش تهدیدکننده زندگی برای بیماران محسوب می‌شود. در این شرایط، خودتعالی به‌عنوان یک بعد قدرتمند انسانی جهت بهزیستی این بیماران ضرورت دارد. مطالعه حاضر با هدف کاربرد نظریه خودتعالی رید (reed’s self-transcendence) در یک بهبودیافته پیوند سلول‌های بنیادی خون‌ساز به‌صورت موردی انجام شد. روش کار: مطالعه حاضر، یک مطالعه موردی با حضور یک دختر 18ساله مبتلا به تالاسمی ماژورِ بهبودیافته از پیوند سلول‌های بنیادی خون‌ساز است که در یکی از مراکز پیوند کشور در سال 1401 انجام گرفت. داده‌ها با استفاده از مشاهده، مصاحبه و مستندات جمع‌آوری و سپس بر اساس نظریه خودتعالی رید تفسیر شد. در این مطالعه، خودتعالی به‌عنوان یک پیامد در فرایند بهزیستی و پرستاری به‌عنوان تعدیل‌کننده این فرایند در نظر گرفته شد.یافته‌ها: یافته‌های این مطالعه بر اساس مفاهیم اصلی نظریه خودتعالی رید شامل آسیب‌پذیری، بهزیستی و رویکردهای مختلف خودتعالی در ابعاد درون‌فردی، بین‌فردی، فرافردی و زمانی مورد تحلیل قرار گرفت. مهم‌ترین این رویکردها در بهبودیافته شامل یادآوری خاطرات و نوشتن یادداشت‌های روزانه، شرکت در گروه‌های اجتماعی و به اشتراک گذاشتن خرد، فعالیت‌های معنوی و افتخار به جنبه‌های مثبت شخصیتی بود. نتیجه‌گیری: بر اساس نتایج مطالعه حاضر، به نظر می‌رسد کاربرد رویکردهای مختلف نظریه خودتعالی رید در بهزیستی بیماران آسیب‌پذیر تحت پیوند سلول‌های بنیادی خون‌ساز تاثیر مثبت داشته باشد. در بسیاری از این رویکردها، رفتارهای مراقبتی پرستاران به‌عنوان تعدیل‌کننده در فرایند خود تعالی محسوب می‌شود؛ بنابراین شناخت و کاربرد این نظریه توسط پرستاران در بیماران تحت پیوند سلول‌های بنیادی خون‌ساز، می‌تواند در دستیابی خودتعالی به این جمعیت آسیب‌پذیر کمک کند. 

from surya to surya: self-transcendence in a survivor case of hematopoietic stem cell transplantation

introduction: hematopoietic stem cell transplantation is a potential treatment but also a life-threatening procedure for patients. in this situation, self-transcendence as a powerful human dimension is necessary for the well-being of these patients. the present study was conducted to apply  reed’s self-transcendence theory in a survivor case of hematopoietic stem cell transplantation.methods: the present study is a case study conducted with the presence of an 18-year-old girl with thalassemia major who recovered from hematopoietic stem cell transplantation, performed in one of iran’s transplant centers in 2023. the data was collected using observation, interviews, and documentation and then interpreted based on pamela reed’s self-transcendence theory. in this study, self-trance was considered as an outcome of the well-being process, and nursing was a moderator of this process.results: the findings of this study were analysed based on the main concepts of reed’s self-transcendence theory, including vulnerability, well-being, and different approaches to self-transcendence in intrapersonal, interpersonal, transpersonal, and temporal dimensions. the most important of these approaches in the survivor included remembering memories and writing diaries, participating in social groups, and sharing wisdom, spiritual activities, and pride in positive aspects of personality.conclusion: based on the results of the present study, it seems that the application of various approaches to reed’s self-transcendence theory has a positive effect on the well-being of vulnerable patients undergoing hematopoietic stem cell transplantation. in many of these approaches, caring behaviors of nurses are considered as moderators in the process of self-transcendence. therefore, knowledge and application of this theory by nurses in patients undergoing hematopoietic stem cell transplantation can help this vulnerable population achieve self-transcendence.
