بررسی تاثیر شان درمانی بر کیفیت زندگی بیماران مبتلا به سرطان: یک کارآزمایی بالینی شاهددار تصادفی شده

مقدمه: بیماران مبتلا به سرطان دامنه ای از علایم شامل درد و انواع ناراحتی های جسمی و روانی را تجربه میکنند که بر کیفیت زندگی این بیماران تاثیر منفی میگذارد. این مطالعه با هدف تعیین تاثیر شان درمانی بر کیفیت زندگی بیماران مبتلا به سرطان انجام شد. روش کار: در این کارآزمایی بالینی 76 بیمار مبتلا به سرطان به روش در دسترس از بین مراجعه کنندگان سرپایی به بیمارستان ایران مهر بیرجند در سال 13971398وارد مطالعه شدند و به صورت تصادفی ساده در دو گروه شان درمانی و کنترل قرار گرفتند.کیفیت زندگی با استفاده از پرسشنامه کیفیت زندگی بیماران مبتلا به سرطان(qlq-c30) قبل و 4 هفته بعد از مداخله شان درمانی مورد سنجش قرار گرفت. تجزیه و تحلیل داده ها با استفاده از نرم افزار spss نسخه 16 و آزمون های t زوجی، ویلکاکسون، t مستقل و من ویتنی و کای اسکوئر انجام شد.یافته ها: میانگین نمرات کیفیت زندگی در ابعاد عملکردی، علایم و کیفیت زندگی عمومی قبل از مداخله بین گروه ها تفاوت معناداری نداشت (0.72=p)، (0.78=p) و (0.70=p)، در حالیکه این میزان بعد از مداخله در ابعاد عملکردی و کیفیت زندگی عمومی در میان دو گروه تفاوت معناداری داشت(0.04=p). قبل و بعد از شان درمانی میانگین نمره بعد عملکردی کیفیت زندگی (0.001>p) و میانگین نمره کیفیت زندگی عمومی در گروه آزمون تفاوت آماری معناداری نشان داد(0.003=p). اما در بعد علایم تفاوت معنی داری مشاهده نشد(0.48=p). میانگین نمرات کیفیت زندگی در ابعاد عملکردی، علایم وکیفیت زندگی عمومی قبل و بعد از مداخله در گروه کنترل تفاوت معناداری نداشت(0.05

The Effect of dignity therapy on quality of life in patients with cancer: A Randomized Controlled Clinical Trial

Introduction: Patients with cancer experience a range of symptoms, including pain and a variety of physical and psychological distress, which negatively affects the quality of life of these patients. The purpose of this study was to investigate the effect of Dignity therapy on quality of life in patients with cancer.Methods: In this clinical trial, 76 cancer patients were studied by available methods among patients referred to Iran Mehr Hospital in Birjand in 13971398 and were randomly assigned to two groups of dignity therapy and control. Quality of life was assessed by Quality of Life QuestionnaireCore 30(QLQC30) at baseline and 4 weeks after the intervention. The data were analyzed in SPSS (version 16) using the paired ttest, Wilcoxon, MannWhitney U, and tdependent tests.Result: Mean scores of quality of life in functional dimension, symptom dimension and general quality of life dimension before intervention were not significantly different between groups (p = 0.72), (p = 0.78) and (p = 0.70), while after the intervention in the functional dimension and the general quality of life dimension was significantly different between the two groups (p = 0.04). Before and after dignity therapy, the mean score of functional of quality of life dimension (p <0.001) and the mean score of the general quality of life dimension in the experimental group showed a statistically significant difference (p = 0.003). But no significant difference was observed in the symptoms (p = 0.48). The mean scores of quality of life in functional dimension, symptom dimension, and general quality of life dimension before and after the intervention in the control group were not significantly different (p >0.05). Conclusion: The results of this study showed that new approaches such as dignity therapy can improve the quality of life of cancer patients. It is recommended that this study be conducted on a larger scale in other hospitals and cultures. It is recommended comparing the effect of this intervention with other psychotherapeutic methods in increasing the quality of life of cancer patients.
