یورتروپلاستی در مادیان؛ روش ها و چالش ها

زمینه و نوع مطالعه: مشکلات دستگاه ادراری-تناسلی در مادیان‌ها می‌توانند تاثیرات منفی بر سلامت و باروری آن‌ها داشته باشند. یکی از این مشکلات، یوروواژینا یا تجمع ادرار در واژن است که می‌تواند منجر به نازایی و عفونت‌های تناسلی شود. این مقاله به‌عنوان یک مطالعه مروری، روش‌های مختلف جراحی یورتروپلاستی را برای اصلاح این اختلالات مورد بررسی قرار داده است.هدف: هدف از این مطالعه، بررسی روش‌های جراحی یورتروپلاستی، شامل تکنیک‌های مختلف امتداد مجرای ادرار، به‌منظور کاهش ناهنجاری‌های دستگاه ادراری مادیان و بهبود سلامت و باروری آن‌ها است. همچنین، این مطالعه به مقایسه این روش‌ها و تحلیل مزایا و چالش‌های هر یک از آن‌ها پرداخته است.روش کار: مطالعه حاضر به‌صورت مروری انجام شده و از منابع معتبر منتشرشده در مجلات و همایش‌های ملی و بین‌المللی، به‌همراه کتب مرجع در زمینه جراحی دام‌های بزرگ، مامایی و تولیدمثل استفاده شده است.نتایج: بررسی روش‌های مختلف یورتروپلاستی نشان می‌دهد که این تکنیک‌ها در اصلاح مشکلات مرتبط با یوروواژینا و بازگشت ادرار به واژن موثر هستند:تکنیک تغییر محل چین عرضی (monin technique) برای موارد خفیف مناسب است و با جابجایی چین عرضی واژن، از ورود ادرار به واژن جلوگیری می‌کند. اما محدودیت‌هایی در اصلاح کامل این مشکل دارد.تکنیک براون (brown technique) شامل ایجاد برش u شکل و امتداد مجرای ادرار است که برای موارد شدید یوروواژینا موثر می‌باشد، اما به دلیل برش مخاطی، خطر خونریزی بیشتری دارد.تکنیک شایرز (shires technique) بدون نیاز به برش، از کشیدن مخاط وستیبول برای ایجاد تونل جدید استفاده می‌کند. این روش کم‌تهاجمی و با حداقل خونریزی است، اما در برخی موارد شدید کارایی کمتری دارد.تکنیک مک‌کینون (mckinnon technique) با ایجاد فلپ‌های مخاطی و بخیه‌زدن آن‌ها برای ساخت یک تونل جدید، روشی بادوام‌تر محسوب می‌شود، اما وسعت برش‌های آن، خونریزی بیشتری را به همراه دارد.نتیجه‌گیری نهایی: یورتروپلاستی یک روش جراحی موثر در درمان ناهنجاری‌های دستگاه ادراری مادیان بوده و به بهبود کیفیت زندگی و افزایش نرخ باروری آن‌ها کمک می‌کند.

background: urinary and reproductive tract disorders in mares can severely impact their health and fertility. one of the major concerns is urovagina, where urine accumulates in the vaginal canal, leading to infertility and reproductive infections. this study reviews urethroplasty techniques for correcting these abnormalities.objectives: this study aims to evaluate various urethroplasty techniques, particularly different urethral extension methods, to address urinary tract abnormalities in mares. additionally, it compares these techniques and analyzes their advantages and limitations.methods: a literature review was conducted using sources from reputable national and international journals, conferences, and reference books on large animal surgery, obstetrics, and reproduction.results: urethroplasty has been shown to be an effective surgical method for addressing urovagina and urine reflux issues in mares. the most commonly used techniques include:monin technique: best suited for mild cases, this method repositions the transverse fold to prevent urine entry into the vagina. while simple, it has limitations in fully correcting the issue.brown technique: involves a u-shaped incision to extend the urethral outlet, making it effective for severe urovagina cases. however, mucosal incisions increase the risk of bleeding.shires technique: a minimally invasive approach that stretches vestibular mucosa without incisions. it reduces bleeding risks but may be less effective for severe conditions.mckinnon technique: uses large mucosal flaps to create a new tunnel, providing a durable solution. however, it involves extensive incisions and a higher risk of bleeding.the success of urethroplasty depends on the severity of the abnormality, the mare's anatomy, and the surgeon's expertise. challenges such as fistula formation, intraoperative bleeding, and suture dehiscence may affect outcomes.conclusions: urethroplasty significantly improves the quality of life and fertility in mares. proper technique selection, patient preparation, and post-operative management are crucial for successful outcomes.
