هفت سال تجربه طرح تحول نظام سلامت در ایران: یک مطالعه تطبیقی و مرور روایتی

مقدمه: پژوهش حاضر با هدف بررسی جنبه های مختلف اجرای طرح تحول نظام سلامت پس از گذشت هفت سال از اجرایی شدن آن در ایران انجام گرفت.روش کار: پژوهش حاضر در دو فاز و در سال 2021 انجام گرفت. در فاز اول، مطالعه تطبیقی اصلاحات و تحولات نظام های سلامت در کشورهای منتخب ترکیه، مالزی، کره جنوبی، مکزیک و برزیل بررسی شد. در فاز دوم در پژوهش با استفاده از روش مرور روایتی، مطالعات منتشر شده در مورد تاثیر طرح تحول نظام سلامت ایران جستجو و تلخیص شد و اطلاعات مربوط به پژوهشگران، استان محل انجام مطالعه و مهمترین یافته های آن‌ها استخراج گردید. تجریه و تحلیل داده ها به صورت مرور جامع مقالات، جمع بندی کیفی و نتیجه‌گیری نهایی انجام شد.یافته ها: تاثیر طرح تحول سلامت بر کارکرد ارائه خدمات در ابعاد زایمان طبیعی و سزارین، رضایت مندی ذی نفعان، ارزیابی عملکرد ارائه دهندگان و حجم و میزان خدمات استفاده شده و تاثیر طرح تحول سلامت بر کارکرد تامین مالی در ابعاد مشارکت مالی خانوار، پرداخت از جیب، عملکرد مالی بخش ها و واحدها، مخارج سلامت و بیمه ها و پرداخت های غیر رسمی شناسایی و بررسی شد.نتیجه گیری: یافته های پژوهش حاضر نشان داد که طرح تحول نظام سلامت در ایران پس از گذشت هفت سال و با اجرای گام ها و بسته های متنوع، در حوزه های کاهش پرداخت از جیب بیماران، نرخ سزارین بدون اندیکاسیون، پرداخت های غیر رسمی و بهره مندی از خدمات سلامت تا حدودی توانسته است به اهداف از پیش تعیین شده دست یابد.

Seven years of experience in the health system transformation plan in Iran: a comparative study and narrative review

Introduction: The aim of this study was to investigate the various aspects of the implementation of the health system transformation plan after seven years of its implementation in Iran.Methods: The present study was conducted in two phases in 2021. In the first phase, a comparative study of health system reforms and developments in selected countries of Turkey, Malaysia, South Korea, Mexico and Brazil was reviewed. In the second phase of the research, using the narrative review method, the published studies on the impact of the Iranian health system transformation plan were searched and summarized, and information about the researchers, the province where the study was conducted, and their most important findings were extracted. Data analysis was performed as a comprehensive review of articles, qualitative summary and final conclusions.Results:The effect of health transformation plan on the performance of services in the dimensions of natural childbirth and cesarean section, stakeholder satisfaction, performance evaluation of providers and the volume and amount of services used and the effect of health transformation plan on the function of financing in the dimensions of household financial participation, payment Out of pocket, the financial performance of departments and units, health and insurance expenses, and informal payments were identified and reviewed.Conclusion: The findings of the present study showed that the health system transformation plan in Iran after seven years and with the implementation of various steps and packages, in the areas of reducing outofpocket payments, cesarean section rates without indication, nonpayments Formalism and access to health services have to some extent been able to achieve predetermined goals.
