ارایه راهنمای عملکردی پرستاران برای برنامه‌ریزی ترخیص بر خودکارآمدی بیماران دیابتی نوع دو

مقدمه: برنامه ریزی ترخیص سبب بهبود خودکارآمدی در بیماران دیابتی نوع دو می شود ولی نحوه عملکرد پرستاران با شرایط موجود در بخش ها همواره چالش اصلی است.هدف: پژوهش حاضر بررسی تاثیر برنامه ریزی ترخیص بر خودکارآمدی بیماران دیابتی نوع دو است که در نهایت بتوان نحوه اجرای انرا به عنوان راهنمای عملکرد پرستاران ارایه داد.مواد و روش ها: پژوهش حاضر یک مطالعه نیمه تجربی و نمونه های پژوهش 70 نفر از بیماران دیابت ملیتوس نوع دو بستری در بیمارستان واسعی سبزوار بودند. مشارکت کنندگان با روش نمونه گیری در دسترس در دو گروه کنترل و آزمون قرار گرفتند. پس از تاسیس دفتر برنامه ریزی ترخیص توسط پژوهشگر، برنامه ریزی ترخیص طی دو مرحله، قبل از ترخیص بیمار از بیمارستان پس از نیازسنجی آموزشی بر اساس مراحل فرایند پرستاری اجرا و سپس بعد از ترخیص به همراه ویزیت در منزل و پیگیری تلفنی به مدت 4 ماه ادامه یافت. ابزار گردآوری اطلاعات چک لیست خودگزارش دهی و پرسشنامه خودکارآمدی در مدیریت دیابت نوع دو (dmses) بود که توانایی بیماران دیابتی را در رعایت رژیم غذایی(8 گویه)، میزان فعالیت بدنی(4 گویه)، مصرف دارو(3 گویه) و اندازه گیری قندخون(4 گویه) می سنجید.یافته ها: آزمون های آماری نشان داد که بین میانگین نمره خودکارآمدی قبل و بعد از مداخله در گروه آزمون(0/000=p) تفاوت معنی داری وجود داشت که در گروه کنترل (0/62=p) در قبل و بعد از مداخله مشاهده نشد. همچنین تفاوت معنی داری در پارامتر فوق (0/000=p)، بین گروه آزمون و کنترل بعد از مداخله وجود داشت. لذا، برنامه طراحی شده مراقبتی دارای اثر بخشی است.نتیجه گیری: به مدیران پرستاری پیشنهاد می شود که با تاسیس دفتر برنامه ریزی ترخیص در بیمارستان برای بیماران دیابتی تداوم و کیفیت مراقبت را تضمین نمایند.

Presentation of nurses' functional guide for discharge planning on self-efficacy of type II diabetic patients

Introduction: Discharge planning improves selfefficacy in type II diabetic patients. But the way nurses work with the conditions in the sectors is always the main challenge.Aim: The purpose of this study was to investigate the effect of discharge planning on selfefficacy of type II diabetic patients, which can be used as a guide to nurses' performance.Material Method: This was a quasiexperimental study with 70 patients with type 2 diabetes mellitus in Vasei Hospital in Sabzevar. Participants were randomly divided into two groups: control and intervention. After the establishment of the discharge planning office by the researcher, discharge planning was performed in two stages before the patient was discharged from the hospital and the educational needs assessment was performed on thebasis of the nursing process and then proceeded after discharge with home visit and telephone follow up For 4 months. The datacollection tool was selfreport checklist and selfefficacy questionnaire on type 2 diabetes management (DMSES). It assessed the ability of diabetic patients to observe diet (8 items), physical activity (4 items), drug use (3 items) and blood glucose (4 items).Results: The statistical tests showed that there was a significant difference between the mean score of selfefficacy before andafter the intervention in the experimental group (P= 0.000), which was not seen before and after intervention in the control group(P = 0/62) . Also, there was a significant difference in the above parameter (P = 0/000) between the experimental and control groups after the intervention. Therefore, the designed care plan is effective.Conclusion: It is recommended that nursing managers ensure continuity and quality of care with the establishment of a hospital discharge planning office for diabetic patients.
