چالش‌های اقتصادی و عملکردی بیمارستان‌های تحت پوشش دانشگاه علوم پزشکی ایران در پی همه‌گیری کووید 19

مقدمه: با توجه به نقش بیمارستان ها در افزایش سطح سلامت جامعه ارزیابی، سنجش و بهبود عملکرد آنها اهمیت ویژه ای دارد. هدف از پژوهش حاضر، بررسی تاثیر همه گیری کووید 19 بر وضعیت اقتصادی و عملکرد بیمارستان های تحت پوشش دانشگاه علوم پزشکی ایران است. روش ها: مطالعه حاضر، کاربردی است که به روش توصیفی تحلیلی در سه بیمارستان غیرتخصصی و با اطلاعات دو مقطع زمانی 1398 و 1400 انجام گردید. اطلاعات مربوط به شش شاخص عملکردی و شش شاخص مالی با مراجعه به واحدهای مدارک پزشکی و مالی بیمارستان ها جمع آوری و از آزمون ناپارامتری ویلکاکسون و نرم افزار اس.پی.اس.اس برای تحلیل داده استفاده شد. یافته ها: نتایج نشان داد که بعد از همه گیری کووید 19، میانگین شاخص های درصد اشغال تخت 12 درصد کاهش و میانگین مدت اقامت و چرخش تخت به ترتیب 12 و 10 درصد افزایش داشته است؛ این تغییرات از نظر آماری معنادار بود (0/05>p value). همچنین، شاخص های مالی درآمدهای اختصاصی آزمایشگاه و تصویربرداری و هزینه بیمارستان ها به ترتیب با 26، 17 و 27 درصد، افزایش معناداری داشتند(0/05>p value). نتیجه گیری: با توجه به تاثیر معنادار همه گیری کووید 19 بر عملکرد مالی و اقتصادی بیمارستان ها و امکان همه گیری مجدد سایر بیماری های واگیر، به نظر می رسد حمایت ویژه از بیمارستان ها در این خصوص ضروری است.

economic and performance challenges caused by the covid 19 epidemic in hospitals affiliated to iran university of medical sciences

introduction: considering the role of hospitals in increasing the health level of society, it is very important to evaluate, measure and improve their performance. the aim of this study was to investigate the impact of the covid 19 epidemic on the economic status and performance of hospitals affiliated to iran university of medical sciences. methods: the present applied study is a descriptive analytical study that was conducted on three non specialized hospitals using data from two periods of time, 2019 and 2020. raw data related to six financial indicators and six performance indicators were collected by referring to the financial and medical records units of the hospitals. the non parametric wilcoxon test and the paired samples t test were used in spss software for data analysis. results: the results showed that after the covid 19 epidemic, the average indicators of bed occupancy rate decreased by 12% and the average length of stay and bed turnover increased by 12% and 10%, respectively; these changes were statistically significant (p value<0.05). furthermore, the financial indicators of dedicated laboratory revenues, dedicated imaging revenues, and hospital expenses increased significantly by 26, 17 and 27%, respectively (p value<0.05). conclusion: considering the significant impact of the covid 19 epidemic on the financial and economic performance of hospitals and the possibility of other epidemic prone emerging infectious diseases, it seems that special support for hospitals is necessary in this regard.
