مقایسه شاخص های عملکردی بخش اورژانس در مراکز تروما

مقدمه: جایگاه اورژانس در کاهش مرگ و میر و تسریع در ارائه خدمات اورژانسی بسیار حساس می باشد. شاخص های عملکردی اورژانس نقشی مهم در ارزیابی عملکرد بخش اورژانس دارند. بنابراین، هدف پژوهش حاضر بررسی شاخص های عملکردی اورژانس در مراکز تروما دانشگاه علوم پزشکی مشهد بود. روش ها: پژوهش حاضر، به صورت ترکیبی و در دو مرحله در سال 1397 انجام شد. در مرحله اول شاخص های عملکردی اورژانس در سه مرکز ترومایی منتخب در سال های 1393تا 1396، جمع آوری گردید. در مرحله دوم به صورت کیفی طی جلسات متمرکز گروهی دلایل تغییرات شاخص ها بررسی شد.یافته ها: طبق بررسی های انجام شده تشکیل پرونده برای بیماران بدون علایم حیاتی، محاسبه درصد احیای قلبیریوی برای کل بیماران ترومایی و غیر ترومایی از جمله عوامل تاثیر گذار بر افزایش درصد احیای قلبی ریوی ناموفق مطرح گردید. افزایش تعداد مراجعین، افزایش تصادفات دسته جمعی، عدم ثبت دقیق زمان به محض ورود پزشک بربالین بیمار، نبود ارتباط سیستم تریاژ با سیستم اطلاعات بیمارستانی از عوامل موثر بر افزایش زمان تریاژ بودند. نتیجه گیری: برای بهبود کیفیت عملکرد بخش اورژانس نیاز به حضور پزشکان متخصص طب اورژانس، تجهیزات کافی، ایجاد کمیته تریاژ و ثبت زمان واقعی تریاژ نیاز است. بنابراین، لازم است تا مدیران این حوزه دستورالعمل ها و فرآیندهای لازم را در جهت افزایش کیفیت عملکرد اورژانس و بازنگری شاخص ها به کار گیرند.

Comparing emergency department performance indicators in Trauma centers

Introduction: The Emergency Department has significant role in reducing mortality and accelerating the delivery of emergency services. Emergency performance indicators play an important role in evaluating the performance of emergency department. Therefore, this study focuses on investigating the emergency performance indicators in Trauma Centers of Mashhad University of Medical Sciences.Method: This was a mixedmethod study conducted in 2018. At first, emergency performance indicators were collected in three selected trauma centers during 20142017. Second, the reasons for the changes in indicators were reviewed qualitatively during the focus group meetings.Results: According to the result, keeping a medical record for patients without vital signs; calculating the cardiopulmonary resuscitation rate for all traumatic and nontraumatic patients increase the rate of unsuccessful cardiopulmonary resuscitation. Some factors were found to increase triage time, such as the increased number of patients and mass accidents, lack of timely recording at the point of care or connection between the triage system and the Hospital Information System.Conclusion: To improve the quality of emergency department performance, an emergency medical practitioners should be present at the emergency department, and adequate equipment, a triage committee and realtime triage records should be available. Therefore, the managers should follow the required guidelines and procedures to improve the quality of the emergency function and review the indicators.
