شناسایی و رتبه‌بندی چالش‌ های نسخه‌ نویسی الکترونیک از دیدگاه ارائه دهندگان خدمت با روش ترکیبی

مقدمه: استفاده از سیستم نسخه‌ نویسی الکترونیک می‌تواند منجر به کاهش خطاهای پزشکی و افزایش ایمنی بیماران شود.هدف پژوهش حاضر، شناسایی و رتبه‌بندی چالش‌های اجرایی نسخه‌نویسی الکترونیک از دیدگاه ارائه دهندگان خدمت در دو بیمارستان تامین اجتماعی استان قزوین بود. روش: این مطالعه با رویکرد ترکیبی در سال 1403 طی دو مرحله انجام شد. در مرحله نخست با رویکرد تحلیل محتوای قراردادی، به منظور شناسایی چالش های اجرایی نسخه نویسی الکترونیک،12 نفر از ارائه دهندگان خدمت با استفاده از نمونه گیری هدفمند و گلوله برفی انتخاب و با آنان مصاحبه‌های نیمه ساختاریافته انجام شد و داده های حاصل با نرم‌افزار maxqda 2020 طبقه بندی شدند. در مرحله دوم، جهت رتبه بندی چالش‌ها با تکنیک topsis، از نرم افزار excel 2021 بهره گرفته شد.یافته‌ها: یافته‌ها نشان داد که چالش های اجرایی شامل 2 طبقه «مسائل زیرساختی» و «مسائل عملکردی» می باشد. در طبقه مسائل زیرساختی، «نقص کدهای خدمات پزشکی» با نزدیکی نسبی 1 بالاترین رتبه و «نواقص بروزرسانی سامانه» با نزدیکی نسبی 0.241 پایین ترین رتبه را داشتند. در طبقه مسائل عملکردی، «افزایش مدت زمان کار با سامانه» با نزدیکی نسبی 0.978 بالاترین رتبه و «افزایش اشتباهات در ثبت نسخه» با نزدیکی نسبی 0.262 پایین ترین رتبه را داشتند.نتیجه‌گیری: با توجه به نتایج، پیشنهاد می شود که سیاست گذاران با اختصاص منابع مالی جهت بازبینی و اصلاح کدهای خدمات پزشکی تعریف شده در سامانه و ساده سازی فرآیندهای سامانه، نسبت به بهبود عملکرد سامانه و ارتقاء رضایت مندی ارائه دهندگان خدمت اقدام نمایند.

identifying and ranking the challenges of electronic prescribing from the perspective of service providers with a mixed approach

introduction: the use of electronic prescription systems can reduce medical errors and enhance patient safety. the present study aimed to identify and rank the implementation challenges of electronic prescribing from the perspective of service providers in two social security hospitals in qazvin province.methods: this mixed-method study was conducted in 2024 in two phases. in the first phase, using a conventional content analysis approach, 12 service providers were selected through purposive and snowball sampling. semi-structured interviews were conducted with the participants to identify the implementation challenges of electronic prescribing. the collected data were categorized using maxqda 2020 software. in the second phase, the topsis technique was applied using excel 2021 to rank the identified challenges.results: the findings revealed that the implementation challenges were classified into two main categories: “infrastructural issues” and “performance issues.” within the category of infrastructural issues, “incomplete medical service codes” ranked highest with a relative closeness of 1, while “system update deficiencies” ranked lowest with a relative closeness of 0.241. in the category of performance issues, “increased time required to work with the system” had the highest rank with a relative closeness of 0.978, whereas “increased errors in prescription entry” had the lowest rank with a relative closeness of 0.262.conclusion: based on the findings, it is recommended that policymakers allocate financial resources to review and revise the defined medical service codes in the system and simplify its processes, in order to improve system performance and enhance service providers’ satisfaction.
