طراحی الگوی اجرای خط‌مشی‌های عمومی سلامت مبتنی بر عدالت

اجرای خط‌مشی‌های عمومی سلامت برپایۀ عدالت یکی از مهم‌ترین اهداف نظام سلامت است؛ اما عواملی مانند شرایط اقتصادی، اجتماعی و سیاسی، و بازیگران بر اجرای خط‌مشی‌ها تاثیر می‌گذارد. تا کنون در کشور الگویی برای اجرای خط‌مشی‌های عمومی سلامت مبتنی بر عدالت طراحی نشده است. این پژوهش براساس نوع داده‌ها، کیفی است و در آن از روش نظریۀ داده‌بنیاد استفاده شده است. داده‌ها از طریق مصاحبۀ نیمه‌ساختاریافته با پانزده نفر از خبرگان سلامت و به کمک روش نمونه‌گیری هدفمند گردآوری شد. کدگذاری نیز مطابق مدل استراوس و کوربین انجام و تحلیل شد. در نهایت داده‌ها با 121 مفهوم، 25 مقولۀ فرعی و 6 مولفۀ اصلی به اشباع نظری رسید. مدل تحقیق شامل پدیدۀ محوری (اجرای خط‌مشی‌های عمومی سلامت مبتنی بر عدالت)، عوامل علّی، عوامل زمینه‌ای، عوامل مداخله‌گر، راهبردها و پیامدهاست. پدیدۀ محوری تحقیق نیز دارای نوآوری است. براساس نظر خبرگان، راهبردهای نوینی، مانند پیوست عدالت سلامت در همۀ سیاست‌ها و حکمرانی خوب سلامت و ایجاد پایگاه اطلاعاتی سلامت، کشف شد که موجب شناسایی پیامدهای جدیدی، مانند مدیریت مالی سلامت، سلامت اجتماعی و عدالت سلامت، شده است.

designing a model for the implementation of publichealth policies based on equity

the implementation of public health policies based on justice is one of the fundamental objectives of health systems. however, various factors, including economic, social, and political conditions, as well as the role of key stakeholders, influence policy implementation. despite its significance, no comprehensive model has yet been developed for the just implementation of public health policies in the country. this study aims to design a model tailored to this purpose. employing a qualitative research approach and grounded theory methodology, data were collected through semi-structured interviews with 15 health policy experts using a purposive sampling method. the analysis followed strauss and corbin’s coding paradigm, leading to theoretical saturation with 121 concepts, 25 subcategories, and six main categories. the proposed model consists of the central phenomenon (the implementation of public health policies based on justice), causal conditions, contextual factors, intervening conditions, strategies, and outcomes. a key innovation of this research is the identification of novel strategies based on expert insights, including integrating a health justice framework into all policies, promoting good health governance, and establishing a comprehensive health database. these strategies contribute to identifying new outcomes such as improved health financial management, enhanced social health, and greater justice in healthcare.
