مطالعه پدیدارشناسانه جستجوی نظریه دوم پزشکی

نظریۀ دوم پزشکی نظریه‌ای است که توسط پزشک دوم دربارۀ نظر پزشک اول و تشخیص و درمانی که به بیمار پیشنهاد داده صادر می‌شود. گرچه کسب نظریۀ دوم ممکن است برای بیمار فوایدی داشته باشد، دلالت‌هایی همچون ناکارآمدی پزشکی مدرن و مشکلات نظام سلامت و پیامدهایی مانند سردرگمی بیماران، استفادۀ بیش‌ازحد از منابع و امکانات و تحمیل هزینه‌های اقتصادی به بیمار و نظام سلامت دارد. بررسی این پدیده از منظری جامعه‌شناسانه می‌تواند در شناسایی ابعاد خرد و کلان و مدیریت آن کمک‌کننده باشد. از این‌رو هدف این مطالعه، شناسایی پدیدۀ نظریۀ دوم پزشکی و عوامل موثر بر آن براساس تجربیات بیماران است.این مطالعۀ پدیدارشناسانه، با تحلیل داده‌های حاصل از مصاحبه با 40 نفر از کسانی که برای یک بیماری اقدام به گرفتن نظریۀ دوم پزشکی کرده بودند، انجام شد.شش عامل بر جست‌وجوی نظریۀ دوم پزشکی موثر هستند: ماهیت بیماری، ویژگی‌های دانش و پراتیک پزشکی، رابطۀ پزشک-بیمار، اتونومی بیمار، عوامل ساختاری (شامل پزشکی تجاری و کالایی‌شدن سلامت، تفاوت سطح توسعه، فقدان نظام ارجاع و بیمه‌های درمانی) و هدایت پزشک اول.پدیدۀ نظریۀ دوم در ایران موضوعی است که تاکنون نادیده گرفته شده و بیماران در نبود هر نوع راهنمایی و پروتکل رسمی براساس ادراکات و منابع خود به پزشکان مختلف مراجعه می‌کنند. برنامه‌ریزی برای هدفمندکردن نظریۀ دوم می‌تواند هم به کاهش سرگردانی بیماران و کاهش هزینه‌های درمانی آن‌ها کمک کند و هم از استفادۀ بیش‌ازحد از منابع و امکانات سلامت پیشگیری نماید.

phenomenological study of seeking a second medical opinion

a second medical opinion is a reevaluation of the diagnosis or treatment provided by a doctor, carried out by a second doctor in the same medical field. although a second opinion may provide the patient with benefits, it also suggests the inefficiency of modern medicine and the problems within the health system. additionally, it may result in patient confusion and the squandering of resources. the study aims to identify, based on the experiences of patients who have sought a second opinion, the phenomenon of of “second medical opinion” and the factors that influence it.the data collected from interviews with 40 individuals who had obtained a second opinion for an illness were used in this phenomenological study.six factors influence the decision to seek a second medical opinion: the nature of the disease, the characteristics of medical knowledge and practice, the doctor-patient relationship, patient autonomy, structural factors (commercial medicine and commodification of health, differences in the level of development, lack of referral system and health insurances), and the guidance of the first doctor.the issue of seeking a second medical opinion in iran has been overlooked thus far. in the absence of any official guidance and protocol, patients seek a second medical opinion based on their perceptions and resources. the planning and administration of the second medical opinion can assist in the prevention of overuse of health resources and facilities, the reduction of patient confusion, and the reduction of treatment costs.
