بررسی علل ریشه‌ای حوادث ناشی از کار در یک شرکت خودروسازی با استفاده از روش Tripod-Beta

مقدمه: حوادث شغلی یکی از چالش‌های مهم موجود در صنایع هستند. هدف از این مطالعه تعیین علل ریشه‌ای حوادث شغلی رخ‌داده در یک شرکت خودروسازی و بررسی نقش آن‌ها در وقوع حوادث است.روش کار: با استفاده از روش tripodbeta حوادث شغلی منجر به استراحتی که سبب از دست رفتن حداقل یک روز کاری در بین سال‌های 1390 تا 1396 شدند، موردبررسی قرار گرفتند و علل تاثیرگذار بر وقوع آن‌ها از سطحی تا ریشه‌ای تعیین گردیدند. تجزیه‌وتحلیل داده‌ها از طریق نرم‌افزار spss22 صورت پذیرفت. یافته ها: حدود80‌‌% حوادث شغلی رخ‌داده در سطح شرکت خودرو‌سازی، در نتیجه 6 عامل ریشه‌ای، شامل نقص در سیستم سازماندهی‌(20‌%)، شرایط تقویت‌کننده‌خطا‌(17‌%)، نقص در سیستم مدیریت تعمیرات و نگهداری‌(12‌%)، نقص در سیستم ارتباطات‌(10‌%)، اهداف ناسازگار‌(10‌%) و نقص در سیستم طراحی‌(9‌%)، به وقوع پیوسته‌اند. عدم وجود اختیار لازم برای متوقف کردن عملیات به میزان 28‌% بیشترین نقش را در بروز نقص سیستم سازماندهی ایفا نموده‌است.نتیجه گیری: با اصلاح سه عامل ریشه‌ای سازماندهی، شرایط تقویت‌کننده خطا و مدیریت تعمیرات و نگهداری، می‌توان بیش از 50 درصد حوادث شغلی را کنترل نمود، جهت کاهش این میزان حوادث، استخدام صحیح نفرات، تعریف دقیق پاسخ‌گویی، تعریف دقیق شرح وظایف، سرپرستی و نظارت مناسب و ثبت شبه حوادث پیشنهاد می‌شود.

Investigation of Root Cause of Work-related Accidents in a Vehicle Manufacturing Company Using Tripod-Beta Method

Introduction: Occupational accidents are one of the major challenges of the industrial workplaces. The identifying of the effective causes of the incidents occurrence, could be used to prevent them. This study was aimed to determine basic causes of occupational accidents in a vehicle manufacturing company.Material and Methods: The occupational accidents leading to loss of time, which cause losing at least one working day, occurred from 2012 to 2017 were analyzed using the TripodBeta method and the causes of their occurrences were determined from the active failures to the root causes. The data were analyzed using the SPSS22 software.Results: Eighty percent of the occupational accidents that occurred in a vehicle manufacturing company were related to 6 root causes, i.e. the weakness of OR (20%), EC (17%), MM not; (12%), CO (10%), IG (10%) and DE (9%). Absence of necessary authority to stop working is the most important reason for the occurrence of the weakness of Organization system with a rate of 28%.Conclusion: By handling three root causes appropriately, i.e. improving OR, EC and MM, more than 50% of accidents can be prevented. Proper hiring of workers, exact definition of accountability and an accurate job description to the employees, proper monitoring and supervision, and nearmiss recordings are suggested to reduce this incidence rate.
