Two-port Laparoscopic-assisted Appendicectomy Under LocalAnesthesia in Adults: A Pilot Study

Aim: nowadays laparoscopic-assisted appendicectomyusing the two-port technique is gaining popularity due to itscertain benefits over the open version. general anesthesiawith positive pressure ventilation is the preferred mode ofanesthesia in this technique. we conducted a pilot study usingthe two-port technique in adult patients, with uncomplicatedappendicitis under local anesthesia, to evaluate its feasibility.materials and methods: in this prospective study 12consecutive patients of asa grade i and ii, with a meanage of 22.5 years, suffering from acute appendicitis, wereincluded. all the patients received ondansetron, diazepam,and pentazocine as premedication. monitored anesthesia carewas given. the site of trocar insertions were infiltrated with 1%lignocaine with adrenalin. the pneumoperitoneum was createdusing carbon-di-oxide. after exteriorization of the appendixusing the trocar, appendicectomy was performed as in theopen procedure. results: eleven out of twelve patients weresuccessfully operated using this method without convertingit into an open method. two cases required an extra port tofree the appendix from the adhesion. there were no intra- orpost-operative complications present. conclusion: two-portlaparoscopic-assisted appendicectomy under local anesthesiais a safe and effective method for uncomplicated appendicitisin adults, and the procedure is suitable where limited set-upfor anesthesia is present.
