تاثیر محیط بالینی حمایتی در دوره کارآموزی بر صلاحیت بالینی دانشجویان پرستاری

مقدمه و هدف: آموزش بالینی نقش مهمی در دستیابی کامل دانشجویان پرستاری به صلاحیت بالینی ایفا می کند. یکی از پارامترهای محیط بالینی که به نظر می رسد در دستیابی به اهداف کارآموزی ها کمک کننده باشد، محیط حمایتی است. پژوهش حاضر با هدف تعیین تاثیر محیط بالینی حمایتی بر صلاحیت بالینی دانشجویان پرستاری طراحی گردید.مواد و روش ها : این مطالعه نیمه تجربی با مشارکت 60 دانشجوی پرستاری که به دو گروه مساوی مداخله و کنترل (با روش قرعه کشی) تخصیص تصادفی شدند، در بیمارستانهای آموزشی مشهد انجام شد. برای دانشجویان گروه مداخله، برنامه محیط بالینی حمایتی که شامل سه جزء دانشی، مهارتی و بستر یادگیری بود، به مدت دو هفته برگزار شد. قبل و بعد از مداخله صلاحیت بالینی آنها با پرسشنامه اصلاح شده صلاحیت بالینی مرتوجا (meretoja modified nursing competence scale) سنجیده شد. داده ها با استفاده از نرم افزار spss 16 و آزمونهای تی مستقل، من ویتنی، تی زوجی و کای اسکوئر تحلیل شدند.یافته ها: نتیجه آزمون تی مستقل نشان داد، بعد از مداخله، میانگین نمره کلی صلاحیت بالینی دانشجویان گروه مداخله(16.6 ±80.8 از 100 نمره) بصورت معنی داری بیشتر از گروه کنترل (10.9 ±68.3 از 100 نمره) بود(0.003 p=). همچنین میانگین نمره صلاحیت بالینی گروه مداخله در ابعاد کمک به بیمار، آموزش و راهنمایی و اقدامات درمانی نسبت به گروه کنترل افزایش معنی داری داشت (0.05p<). نتیجه گیری: ایجاد محیط بالینی حمایتی، صلاحیت بالینی دانشجویان را خصوصا در حیطه های کمک به بیمار و آموزش و راهنمایی بیشتر ارتقا می دهد و توصیه می شود در برنامه ریزی آموزش بالینی پرستاری توجه بیشتری به آن شود.

The Effect of Supportive Clinical Environment in internship period on Clinical Competence of Nursing Students

Introduction: Clinical education accounts for about 50% of nursing education programs and plays an important role in the full achievement of clinical nursing students, so they should change the environment in a way that facilitates students #39; learning and education. The purpose of this study was to determine the effect of supportive clinical environment in internship period on clinical competence of nursing students.Methods and materials: This randomized qusi experimental study was carried out with 60 nursing students who allocated in two groups of intervention and control randomly in educational hospitals of Mashhad. For interventional students was performed the clinical support program, which included three components of knowledge, skills, and learning platform for two weeks. Before and after the intervention, their clinical competency was measured by Meretoja modified clinical competence scale. Data were analyzed by SPSS software version 16 and independent ttest, MannWhitney, paired Ttest and Chisquare.Results: The result of independent ttest showed that after the intervention, the mean score of clinical competency of the students in the intervention group (80.8 ± 16.6 of 100) was significantly (68.3 ± 10.9 of 100) more than the control group (p= 0.003). Also the mean score of clinical competency of the intervention group in the dimensions of patient assistance, education, guidance and therapeutic measures was significantly higher than that of the control group (P <0.05).Conclusions: Creating a supportive clinical environment will enhance the clinical competence of the students, especially in the areas of patient assistance and further education and guidance, and it is recommended that more attention be paid to planning clinical nursing education.Key words: Clinical Education, Support, Nursing Process, Clinical competence, Nursing Student
