چکیده
|
زمینه و هدف ها: پروتئوس میرابیلیس یکی از عوامل شایع در عفونت های مجرای ادراری (uti) و گاهی باکتریمی میباشد. هدف از مطالعه حاضر شناسایی مولکولی ژن های inti، intii و sul در سویه های پروتئوس میرابیلیس جدا شده از ادرار بوسیله multiplexpcr و بررسی مقاومت آنتی بیوتیکی این سویهها میباشد. مواد و روش ها: در این مطالعه توصیفیمقطعی، تعداد 292 نمونه ادرار از بیماران دارای عفونت ادراری بدست آمد. آزمون حساسیت آنتی بیوتیکی با استفاده از روش انتشار از ژل و مطابق دستورالعمل استاندارد آزمایشگاه و بالین (clsi) بر روی محیط مولر هینتون آگار انجام شد. سپس multiplexpcr برای شناسایی ژن های inti، intii و sul با استفاده از توالی های الیگونوکلئوتیدی ویژه انجام شد. یافته ها: نتایج تست های روتین بیوشیمیایی و میکروبیولوژی نشان داد که از مجموع 292 نمونه ادرار تحت مطالعه، 60 نمونه پروتئوس میرابیلیس بدست آمد. بیشترین میزان حساسیت به آمیکاسین (100%) و جنتامایسین (98.3%) مشاهده شد. همچنین 10% سویهها به سیپروفلوکساسین مقاوم بودند. نتایج آزمون مولکولی نشان داد که بیشترین و کمترین شیوع به ترتیب مربوط به ژن های intii وsul1 با پراکندگی 81.2% و 25% بود.نتیجه گیری: با توجه به نقش اینتگرون در گسترش فاکتورهای مقاومتی و فراوانی مقاومت آنتی بیوتیکی در سویه های پروتئوس میرابیلیس، توجه به مدیریت صحیح در مصرف آنتیبیوتیکها و اتخاذ تدابیر لازم در رعایت استانداردهای بهداشتی میتواند در جلوگیری از گسترش مقاومت موثر باشد.

Molecular Identification of Integron and Sulfonamide Resistance Genes in the Proteus Mirabilisis Isolated from Urine Samples by Multiplex-PCR and Their Antibiotic Resistance Profile

Abstract
|
Introduction: Proteus mirabilis is a common pathogen responsible for complicated urinary tract infections (UTIs) that sometimes causes bacteremia. The aim of the study was, Molecular identification of virulence genes at the same time intI, intII and sul in Proteus mirabilisis from urine MultiplexPCR and Swarming Test.Methods: In this crosssectional study, 292 urine samples were collected. Antimicrobial susceptibility test was performed by disk diffusion test according to the clinical laboratory sard institute test (CLSI) on the Muller Hinton agar. Then, multiplexPCR was achieved for determination of intI, intII and sul genes in the strains by specific oligonucleotides primers.Results: Of 292 urine specimens, 60 P. mirabilis were obtained by standard microbiological and biochemical tests. The antimicrobial susceptibility result showed that the highest susceptibility rate were related to amikacine (100%) and gentamicin (98.3%). so, 10% of strains were resistance to ciprofloxacin. Distribution of these genes showed that 81.2% and 25% isolates were carring intII and sul1, respectively.Discussion and Conclusion: Considering the role of integron in the development of resistance factors and the frequency of antibiotic resistance in Proteus mirabilis strains, paying attention to proper management in the use of antibiotics and taking necessary measures in compliance with health standards can be effective in preventing the spread of resistance.
