پوزیشن های زایمان طبیعی از منظر طبیبان متقدم (قرن‌های 3-13)

مقدمه: زایمان و مادر شدن از مهم‌ترین وقایع در زندگی هر کس است. از زمان‌های گذشته در تمامی تمدن‌ها از وضعیت قرارگیری زائو به عنوان راهی برای تسهیل و تسریع زایمان استفاده شده است و یکی از تدابیر مهم و موثر در روند لیبر و زایمان است. نظر به سابقه غنی طب سنتی و راهبردی بودن دستورات آن، می‌توان انتظار داشت که با شناسایی وضعیت‌های قرارگیری زائو که در منابع کهن طب سنتی ارائه شده، بتوان گام مهمی در پیشگیری از طولانی شدن روند زایمان، دشوارزایی و در نهایت کاهش میزان سزارین و افزایش سلامت مادر و نوزاد برداشت. روش کار: پژوهش حاضر یک مطالعه‌ی کتابخانه‌ای می‌باشد که در آن متون معتبر طب سنتی شامل کفایه الطب، الحاوی، قانون، مفرح القلوب، خلاصه الحکمه و یازده کتاب دیگر مورد مطالعه‌ی مستقیم قرار گرفته و مطالب مرتبط با وضعیت قرارگیری زائو جهت تسهیل زایمان جمع آوری و با رعایت اصل امانت داری ارائه گردید. یافته‌ها: در مطالعه‌ی حاضر در راستای تسهیل زایمان، به وضعیت‌های چمباتمه، نشسته، سجده، ایستاده و نیمه نشسته اشاره شده است و جهت افراد چاق یا دارای توده‌های لگنی و شکمی، وضعیت سجده توصیه و برای موارد انحراف بدن جنین به سمت چپ یا راست و قدام یا خلف، قرارگیری زائو در وضعیت مخالف با سمت انحراف یا اصلاح با دست ذکر شده است و در نهایت بهترین وضعیت قرارگیری جهت زایمان، نشستن روی صندلی معرفی شده است. نتیجه گیری: در متون طب سنتی نه تنها به انواع پوزیشن های مورد استفاده در پزشکی رایج، اشاره شده است بلکه پوزیشن های جدیدی مثل وضعیت سجده برای افراد چاق، حرکت مشابه تاب سواری، پایین و بالارفتن از پله و نشست و برخاست ناگهانی نیز معرفی شده است که با انجام تحقیقات بیشتر در این زمینه به نظر می‌رسد می‌توان از آنها برای تسهیل و تسریع زایمان و رفع دشوارزایی استفاده نمود.

natural delivery positions according to ancient doctors of the islamic civilization period (the 3rd to 13th centuries)

introduction: childbirth and becoming a mother is one of the most important events in everyone's life. since the past times, in all civilizations, the positions has been used as a way to facilitate and speed up childbirth and it is one of the important and effective measures in the process of labor and delivery.it can be expected due to the history of traditional medicine and the strategic nature of its orders wich by identifying the positions which are presented in old texts of traditional medicine, it is possible to take an important step in preventing the prolongation of the birth process, dystocia and finally reducing the rate of cesarean section and increasing the health of mother and baby. methods: the current research is a library study in which the authentic texts of traditional medicine, including kefayat al teb, al-havi, al-qanun, mofarreh al-qolub, kholase-alhekmah and eleven other books, are directly studied and materials related to the mother's position were collected to facilitate childbirth and were presented in compliance with the principle of confidentiality. results: in the present study squatting, sitting, knee chest, standing and semi sitting positions have been mentioned and knee chest position is recommended for obese people or those with pelvic and abdominal masses. in cases of deviation of the fetal body position to the left or right and anterior or posterior, placing the position opposite to the side of the deviation or correction by hand is mentioned and finally, the best position for childbirth is sitting on a chair. conclusions: in the texts of traditional medicine, not only the types of positions used in common medicine are mentioned, but also new positions such as knee chest position for obese people, the movement similar to swing riding, going down and going up the stairs and sitting down and standing up suddenly has been introduced which by doing more research in this field, it seems that they can be used to facilitate and speed up childbirth and to eliminate dystocia.
