ارزیابی مهارت ارتباطی گفتمان در بیماران آسیب ‌دیده نیمکره چپ و راست فارسی زبان

مطالعه حاضر با هدف ارزیابی و مقایسه اختلالات گفتمانی در بیماران بزرگسال آسیب دیده نیمکره‌ راست و چپ مغز انجام گرفت . جامعه آماری پژوهش شامل 10 فرد بزرگسال فارسی زبان آسیب دیده نیمکره‌ چپ و نیمکره‌ راست است که دچار سکته مغزی شده اند. ابتدا آزمون هایی برای پایش آزمودنی های بیمار از نظر زوال عقل ، سوبرتری و وجود یا عدم وجود زبان پریشی انجام شد و سپس با استفاده از خرده آزمون ها ی پروتکل ارز ی ابی مهارت های ارتباطی مونترال (ام. ای. سی)، پردازش گفتمان آزمودنی ها را مورد ارزیابی قرار دادیم. یافته ها نشان دادند که بین عملکرد دو گروه بیماران در مهارت ارتباطی گفتمان تفاوت وجود دارد، امااین تفاوت عملکرد معنی دار نیست. در گروه بیماران آسیب دیده نیمکره راست ، بیمارانی با آسیب لوب گیجگاهی، ضعیف ترین عملکرد را در مهارت های ارتباطی گفتمان داشتند که نشان می داد آسیب به لوب گیجگاهی و نواحی پیرامون آن در نیمکره راست مغز می تواند منجر به بروز اختلال در مهارت گفتمانی شود. بررسی رابطه جایگاه آسیب مغزی و میزان اختلالات گروه بیماران آسیب دیده نیمکره چپ نیز دلالت بر آن داشت که آزمودنی هایی با جایگاه آسیب در نواحی گیجگاهی - پیشانی و گیجگاهی -آهیانه ای در نیمکره چپ مغز، بالاترین میزان اختلال عملکرد را نشان دادند. آسیب به هر دو نیمکره راست و چپ مغز منجر به بروز اختلال درمهارت گفتمانی می شود، اما آسیب به نیمکره راست با شدت اختلالات بیشتری همراه است. بر پایه داده هایی که از دو گروه بیماران پژوهش حاضر به دست آمد، می توان ادعا کرد که بین بروز اختلال در پردازش مهارت گفتمانی با جایگاه ضایعه مغزی رابطه مستقیمی وجود دارد.

the evaluation of discourse communication skill of right and left hemisphere damaged persian-speaking patients

this paper draws on to evaluate one of the communication skills, discourse, in adultright and left hemispheres damaged persian speaking patients due to cerebrovascular accident (cva) using the persian version of the montreal protocol for the evaluation of communication (m.e.c.). the statistical populations of the study were 10 adults, rightand left hemispheres damaged persian speaking patients due to cerebrovascular accident(cva). two sets of tests were used in the present study. the first ones were screening tests included lateral preference invent ory (corn test), mini mental state examination (mmse) and persian diagnostic aphasia battery (eq1). these tests were performed tomonitor the subjects in terms of handedness, dementia and the presence or absence ofaphasia. the second set of tests were the persian version of the montreal protocol for the evaluation of communication, which was used for assessing the communicative skills. the performance of patients in protocol subtests were evaluated and comparedwith each other individually, intragroup and intergroup. findings indicated that damage to the right and left hemispheres of the brain leads to discourse impairments. however, damages to the right hemisphere increases the severity of linguistic impairments more than damages to the left hemisphere. all patients who had the weakest performance in the sub-tests of discourse protocol tasks, their temporal lobe was damaged. in rhd,those patients who had the weakest performance in the sub-tests of protocol skills, theirdamages were located in the temporal lobe. as a result, it seems that damage to the temporal lobe in the right hemisphere of the brain leads to impaired discourse skills. in lhd group, damages to the frontotemporal and temporal-parietal areas in the lefthemisphere of the brain, cause some impaiments to the discourse skills of lhd patients. therefore, based on these datas, it may be claimed that there is a direct relationship between the degree of impairments of discourse and pragmatic skills and the location ofthe brain lesion.
