ارزیابی خطای انسانی به روش Heart در مشاغل اتاق های عمل بیمارستان امام حسین (ع) شهرستان گلپایگان و ارائه ی راهکارهای اولویت بندی شده

مقدمه: با توجه به اهمیت مشاغل درمانی و علی الخصوص مشاغل موجود در اتاق عمل در بهبود بیماری ها و همچنین حفظ جان افراد ، بروز خطا و قصور در انجام وظایف کارکنان این مراکز تبعات جبران ناپذیری خواهد داشت لذا یافته های این مطالعه می تواند راهنمای مناسبی جهت کاهش خطاهای کارکنان اتاق عمل باشد. روش بررسی: این مطالعه به روش heart جهت بررسی خطای انسانی کارکنان اتاق عمل انجام شد.جهت انجام این مطالعه ضمن حضور در شیفت های مختلف در اتاقهای عمل و آموزش و توجیه کارکنان این بخش در خصوص مطالعه و و اهداف آن، مراحل انجام کار کارکنان اتاق عمل در شغل های مختلف مشاهده شد و اسناد و دستورالعمل های موجود بررسی گردید.همچنین گام های اصلی که شامل آنالیز سلسله مراتبی مشاغل،شناسایی خطاها برحسب هر وظیفه و زیر وظیفه،تکمیل سایر قسمتهای برگه کار و تعیین احتمال بروز خطای انسانی و ارایه ی راهکارهای کنترلی اولویت بندی شده در مراحل بعد انجام شد.نتایج: عامل خستگی با فراوانی 164 (11.47%) و سهل انگاری با فراوانی 160 (11.2%) بیشترین عوامل بروز خطا در مشاغل اتاق های عمل این بیمارستان شناسایی شدند و محیط کار نامناسب با فراوانی 1 (0007/%)کمترین عامل بروز خطا می باشد. نتیجه گیری:تجهیز اتاق های عمل به تجهیزات پیشرفته، به کارگیری و استخدام نیروهای ماهر و باتجربه، تنظیم برنامه ی کار و استراحت کارکنان، ارائه ی آموزش های متناسب با نیازهای کارکنان و بهبود سیستم های نظارتی و مدیریتی مهمترین راهکارهای بهبود شرایط موجود و کاهش خطاهای انسانی می باشد.

Considering the importance of medical care and especially occupations in the operating room for the improvement of illnesses as well as the preservation of life, the consequences of failure and failure to perform the duties of the staff of these centers will be irreparable, so the findings of this study can be a useful guide to reduce Errors in operating room staff.Methods: This study was performed by HEART method to investigate the human error in the operating room staff. To perform this study, presenting various shifts in the operating rooms, training and justifying the employees of this department regarding the study and its objectives, the staffing process The operating room was observed in various occupations and the existing documents and guidelines were reviewed. Also, the main steps, including hierarchical analysis of occupations, identifying errors in terms of each task and sub task, completing other parts of the work sheet and determining the probability of human error and providing Prioritized control strategies were carried out in the next steps.Results: According to the results, Fatigue factor with frequency of 164 (11.47%) and negligence with frequency of 160 (11.2%) were the most frequent errors in the occupations of operation rooms of this hospital. The inadequate work environment with frequency 1 (/0007%) The least factor is the error.Conclusion: equipping the operating rooms with advanced equipment, employing and recruiting skilled and experienced personnel, adjusting the work schedule and resting the staff, providing training tailored to the needs of the staff, and improving the supervisory and management systems.
