Background: endometrial scratch injury is considered controversial in increasing the success rate of assisted reproductive technology.objective: to compare the pregnancy outcomes in women candidated for intrauterine insemination with and without an endometrial scratch.materials and methods: in this randomized clinical trial, 150 women referred to the fatemieh hospital, hamadan, iran who were candidates for iui between december 2017 to december 2018 were randomly assigned into two groups (n = 75/each) with or without an endometrial scratch (as case and control groups, respectively). women in both groups were in the proper and identical protocol for iui. chemical and clinical pregnancy, abortion, and live birth rate, also pregnancy complications were compared between the groups.results: chemical and clinical pregnancy rates were higher in the case than the control group (p = 0.25, p = 0.54, respectively). in the case group, the abortion and multiple gestation rates were 14.3% and 4.3%, respectively, while it was 5% in the control group (p = 0.60, p = 0.54 respectively). the endometrium thickness on day 21 was higher in the case group than the control (p = 0.01).conclusion: endometrial scratching in intrauterine insemination women is not associated with an increase in both clinical and clinical pregnancy rates, however, studies with a larger sample size are suggested to evaluate this intervention.

Abstract
|
مقدمه: انجام خراش آندومتر برای افزایش میزان موفقیت در روش های کمک باروی بحث برانگیز است.هدف: هدف از انجام این مطالعه بررسی مقایسه اثر بخشی پیامد حاملگی در بیماران کاندید تلقیح داخل رحمی با و بدون خراش آندومتر می باشد.مواد و روش ها: در این کارآزمایی بالینی تصادفی، 150 زن مراجعه کننده به بیمارستان فاطمیه همدان که کاندید تلقیح داخل رحمی بودند به طور تصادفی در دو گروه 75 نفر با و بدون خراش آندومتر قرار گرفتند. بیماران هر دو گروه در پروتکل مناسب و یکسان شدند. حاملگی شیمایی، بالینی، سقط، تولد زنده و عوارض حاملگی در هر دو گروه مقایسه شد.نتایج: میزان حاملگی شیمیایی و بالینی در گروه مورد بالاتر از شاهد بود (به ترتیب 25/0 و 54/0 = p). سقط جنین و میزان حاملگی چندقلویی در گروه مورد به ترتیب 3/14% و 3/4% بود، که این میزان در گروه کنترل 5% برآورد شد. (به ترتیب 54/0 و 60/0 = p) ضخامت آندومتر در روز 21 در گروه مورد بالاتر از گروه شاهد بود (01/0 = p).نتیجه گیری: خراش آندومتر در زنان تلقیح داخل رحمی با افزایش میزان حاملگی شیمایی و بالینی همراه نمی باشد، با این حال مطالعات با حجم نمونه بیشتر برای تأیید یا رد این مداخله پیشنهاد داده می شود. کلمات کلیدی: خراش آندومتر، تزریق درون رحمی اسپرم، حاملگی.
