Luteinizing Hormone Changes in Gonadotropin-Releasing Hormone Antagonist Protocol in In Vitro Fertilization Cycles: A Cross-Sectional Study

Background: because of the unexpected and often dramatic inhibition of luteinizing hormone (lh) secretion related with the usage of gonadotropinreleasing hormone (gnrh)antagonist, there has been a probable need for exogenous lh supplementation. there is a basic and clinical evidences that show late development of follicle needs an lh but there is a threshold for lh requirements during folliculogenesis.objective: the purpose of this study was to evaluate the changes in serum lh and the identification of patients who benefit from the addition of lh. materials and methods: seventy volunteers for antagonist protocol in ivf cycle were enrolled in this prospective crosssectional study. the study was carried out in reproductive health research center, university of medical sciences between july 2016 and february 2016. serum lh level was estimated 24 h before and after the first (gnrh) antagonist injection. the primary outcome was the serum level of lh and its change in the three groups and the secondary outcome was egg and embryo quality.result: lh changes above or below 50% had no effect on the number of follicle, the number of oocyte, germinal vesicle oocyte, metaphase 1 oocyte, metaphase 2 oocyte, endometrial thickness, and chemical and clinical pregnancy.conclusion: we evaluated the changes of serum lh in the patients who were entered in the antagonist protocol. our study showed no significant difference in lh levels 24 h before and after the injection of the antagonist between the three groups, and lh changes did not affect the outcome of pregnancy.

تغییرات LH (هورمون لوتئینیزان) در برنامه درمانی آنتاگونیست GnRH در سیکل‌های لقاح آزمایشگاهی (مطالعه مقطعی آینده‌نگر)

مقدمه: نقش LH در فولیکولوژنز شامل تحریک سلول تکا, تولید اندروژن و حمایت از جسم زرد و تکمیل مرحله نهایی فولیکولار می باشد. یکی از مشکلات در استفاده از آنتاگونیست GnRH کاهش میزان LH می باشد.هدف: هدف از این مطالعه ارزیابی سطح سرمی LH در بیماران استفاده کننده از آنتاگونیست GnRH در سه گروه از بیماران (پاسخ نرمال, پاسخ کم و پاسخ زیاد) می باشد.موارد و روش ها: هفتاد بیمار داوطلب سیکل آنتاگونیست در برنامه درمانی لقاح آزمایشگاهی در این مطالعه مقطعی آینده نگر وارد شدند. این مطالعه در مرکز تحقیقات باروری دانشگاه علوم پزشکی تهران در فاصله زمانی انجام شد. سطح سرمی LH یک روز قبل و بعد تزریق آنتاگونیست ارزیابی شد. پیامد اولیه میزان سرمی LH و پیامد ثانویه کیفیت تخمک و جنین بود.نتایج: بر اساس تست من ویتنی و فیشر تغییرات بیشتر و کمتر از پنجاه درصد LH تاثیری بر تعداد فولیکول, تخمک GV (ژرمینال وزیکول), MI (متافاز یک), MII (متافاز دو), ضخامت آندومتر نداشت. (نتایج در جدول 3 آورده شده است).نتیجه گیری: ما سطح سرمی LH را در بیماران تحت سیکل آنتاگونیست ارزیابی کردیم. ما تغییرات را در سه گروه (پاسخ دهندگان ضعیف, پاسخ دهندگان قوی و پاسخ دهندگان نرمال) بررسی کردیم. طبق مطالعه ما تغییرات سرمی LH قبل و بعد تزریق آنتاگونیست چشمگیر نبود و تفاوتی در پیامد های بارداری نداشت. بنابراین عملا لزومی به اضافه کردن LH در سیکل های آنتاگونیست نمی باشد.
