بررسی اثر پروپوفول و ایزوفلوران بر شیوع لرز پس از عمل جراحی ویتره رتین چشم

زمینه و هدف: لرز یکی از شایع ترین مشکلات بیهوشی است که می تواند سبب افزایش فشارخون، افزایش مصرف اکسیژن، افزایش فشار داخل چشم و افزایش ضربان قلب در بیماران گردد. از آنجایی که انتخاب داروهای استنشاقی و وریدی موثر در کاهش لرز پس از عمل جراحی ضروری است، بنابراین هدف این مطالعه مقایسه ی اثرات بیهوشی با ایزوفلوران و پروپوفول بر شیوع لرز پس از جراحی ویتره رتین چشم می باشد.روش بررسی: در یک مطالعه کارآزمایی بالینی، 112 بیمار 7516 سال با خطر بیهوشی کلاس i-iii انجمن متخصصان بیهوشی آمریکا داوطلب جراحی ویتره رتین چشم به طور تصادفی به دو گروه 56 نفره تقسیم شدند. وقوع لرز پس از عمل جراحی در دو گروه در اتاق ریکاوری ثبت شد. یافته ها با آزمونهای تی و مجذورکای دو تجزیه و تحلیل شدند و مقدارp کمتر از 0/05 در نظر گرفته شد.یافته ها: بین دو گروه از نظر سن، جنس، وزن، مدت جراحی و زمان بیهوشی تفاوت معنی داری وجود نداشت. بروز لرز در گروه ایزوفلوران 12/5% و در گروه پروپوفول ورمی فنتانیل 32/1% بود.نتیجه گیری: استفاده از ایزوفلوران در کاهش بروز لرز پس از جراحی ویتره رتین چشم، بهتر از پروپوفول است.

The Evaluation Between The Impact Of Using Isoflurane And Propofol On Shivering Among Patients Undergoing Vitrectomy Surgery

Background and Aim: The postoperative shivering is one of the common complications of anesthesia, which can cause increased systemic blood pressure, oxygen consumption, intra optical pressure and heart rate in patients. It is necessary to choose an effective intravenous and the inhaled drugs to reduce postoperative shivering, therefore the aim of this study was to compare Isoflurane and Propofol on shivering among patients undergoing Vitrectomy Surgery.Materials and Methods: In a doubleblind clinical trial, 112 patients aged 16 to 75 (ASA class IIII) who underwent the Vitrectomy surgery were randomly classified into two groups of 56 cases. The occurrence of postoperative shivering in recovery were registered in both groups. Findings were analyzed employing the ttest and Chisquare. Statistical significance was accepted for P values of <0.05.Results: There was no significant difference between the two groups in terms of age, sex, Weigh، duration of surgery and duration of anesthesia. The incidence of shivering in recipients of Isoflurane and in recipients of propofol was 12.5% and 32.1% respectively.Conclusion: Using Isoflurane is better than Propofol to reduce the postoperative shivering in patients undergoing the vitrectomy surgery.
