هویت بدنی در زنان مبتلا به سرطان پستان

زمینه و هدف: درمان سرطان پستان با تغییراتی در بدن زنان همراه است. تغییراتی که علاوه بر جنبه‌ی ظاهری با زنانگی آنان در ارتباط است و می‌تواند بخشی از هویت زنان که در ارتباط با بدن آنان است را با چالش‌هایی مواجه سازد. این پژوهش به فرایند تغییرات هویت‌بدنی زنان در بستر فرهنگ و اجتماع می‌پردازد.روش بررسی: از روش کیفی برای انجام پژوهش استفاده شده است و بدین‌منظور مصاحبه‌هایی عمیق و نیمه ساختاریافته با 15 زن تهرانی مبتلا به سرطان پستان که مراحل درمان را طی کرده بودند، 5 نفر از همراهان که در مراحل بیماری در کنار زنان مبتلا بودند و 3 پزشک جراح انجام گرفت. در مصاحبه‌ها به تجربیات زنان، احساسات و اقدامات آنان در مواجهه با تغییرات بدنی پرداخته شد. نمونه‌گیری به‌صورت نمونه‌گیری هدفمند و گلوله‌برفی انجام شد. جهت تحلیل مصاحبه‌ها از تکنیک تحلیل تماتیک braun و clarke استفاده گردید.یافته‌ها: زنان مشارکت‌کننده 27 تا 65 ساله بودند که 8 نفر از آنان مدرک کارشناسی و بالاتر داشتند. 7 زن خانه‌دار و 8 زن شاغل بودند و 13 زن ماستکتومی انجام داده بودند. مضامین اصلی به‌دست آمده شامل تغییر در بدن زنانه، چالش با جامعه، اختلال در هویت‌بدنی، حمایت و همراهی، تنگنای اقتصادی و بازتعریف هویت‌بدنی است. این مضامین فرایند مواجهه‌ی زنان با تغییرات بدنی را توضیح می‌دهد. زنان پس از تغییرات بدنی، چالش‌هایی با جامعه دارند. جامعه بدن زنان را پس از تغییرات قضاوت می‌کند و نگاه منفی به آن دارد. از سوی دیگر زنان از بدن خود احساس نارضایتی دارند و بدن خود را برای انجام وظایف زنانه و ایفای نقش همسری و مادری ناکارآمد می‌دانند. اما با گذشت زمان، زنان با پذیرش تغییرات و مدیریت بدن، تلاش می‌کنند تا هویت‌بدنی خود را مجدداً بازسازی نمایند. علاوه بر این وضعیت اقتصادی زنان و همراهی و حمایت اطرافیان که به‌صورت تایید تغییرات بدن زن و همراهی و حمایت عاطفی است، می‌تواند پذیرش تغییرات بدنی را برای زنان امکان‌پذیرتر نماید.نتیجه‌گیری: با توجه به اهمیت بدن در تعریف زنانگی، زنان پس از ابتلا به سرطان پستان علاوه بر رنج بیماری با چالش‌های هویتی نیز مواجه هستند. پس درمان سرطان پستان در کنار اقدامات بالینی، نیازمند آگاهی جامعه از نحوه‌ی مواجهه با زنان بیمار است.

background and aim: breast cancer treatment is associated with changes in women’s bodies. changes that are related to their femininity in addition to the appearance aspect and can face challenges in the part of women’s identity that is related to their body. this research deals with the process of changes in women’s physical identity in the context of culture and societymaterials and methods: a qualitative method was used for the research, and in this regard, in-depth and semi-structured interviews were conducted with 15 women from tehran who had undergone treatment for breast cancer, along with 5 companions who were alongside the patients during their illness, and 3 surgical doctors. the interviews focused on the experiences, emotions, and actions of the women in response to bodily changes. sampling was conducted through purposive and snowball sampling methods. the thematic analysis technique developed by braun and clarke was employed for analyzing the interviews.results: the participating women were aged 27 to 65 years, with 8 holding bachelor’s degrees or higher. seven women were housewives, 8 were employed, and 13 had undergone mastectomies. the main themes identified include changes in the female body, societal challenges, disruption of body image, support and companionship, economic constraints, and the redefinition of body image. these themes explain the process of women’s coping with bodily changes. following bodily changes, women face challenges from society. society judges women’s bodies after these changes and views them negatively. furthermore, women experience dissatisfaction with their bodies, perceiving them as inadequate for fulfilling feminine roles and responsibilities as wives and mothers. however, over time, through acceptance of the changes and body management, women strive to reconstruct their body image. in addition, the women’s economic situation and the support and companionship of those around them—manifesting as acceptance of the bodily changes and emotional support—can facilitate the acceptance of these changes.conclusion: given the importance of the body in defining femininity, women, after experiencing breast cancer, face not only the suffering of the disease but also identity challenges. therefore, breast cancer treatment, alongside clinical interventions, requires societal awareness of how to interact with affected women.
