چکیده
|
صرع اختلال سیستم عصبی مرکزی (اختلال نورولوژیکی) است که در آن فعالیت سلولهای عصبی در مغز، مختل و به تشنج منجر میشود. بیشتر افراد مبتلا با استفاده از داروهای مناسب حملات خود را کنترل میکنند؛ اما متاسفانه همیشه درمان دارویی پاسخگو نیست و عدهای از این افراد بهناچار درمان جراحی را میپذیرند. مسئلۀ اصلی در این نوع جراحی و یا هر نوع جراحی دیگر مغز که در آن لازم باشد جراح بخشهایی از بافت مغز را تخریب و یا خارج کند، خودداری از تخریب بافتهای سالم و حیاتی نزدیک محل جراحی است. درواقع شناسایی محل دقیق کانونهای صرع موجود در کورتکس بسیار مهم است؛ بنابراین نظر به اینکه عامل موفقیت در درمان این بیماری تعیین دقیق کانون مولد صرع است، الگوریتمهای مختلفی برای مکانیابی منابع مغزی و درنتیجه تعیین دقیق کانون مولد صرع ارائه شده است؛ ولی تا کنون هیچیک نتوانستهاند راهحل مناسبی برای حل این مشکل در دنیای پزشکی ارائه دهند. با توجه به اینکه سیگنال eeg، اطلاعات زمانی مناسب و fmri، اطلاعات مکانی دقیقتری دارند، امید است با ترکیب دو مدالیته عملکرد بهتری به دست آید. در این مقاله در ابتدا سعی شده است آرتیفکتهای موجود حاصل از گرادیان روی eeg، حذف و با مکانیابی منابع مغزی، سیگنال درون اسکنر و خارج آن مقایسه و ارزیابی شود. سپس با پردازش همزمان eeg-fmri و استفاده از قیودی مستخرج از اطلاعات زمانی eeg به تحلیل fmri و مکانیابی کانونها با مدل glm پرداخته شده است. مطالعه بر 10 بیمار صرعی موضعی مقاوم به دارو انجام شد که در مرکز نقشهبرداری مغز ایران از آنها ثبت داده شدهاند. نتایج روش پیشنهادی نسبت به روشهای ارائهشده تا کنون حاکی از بهبود چشمگیر درستی مکانیابی است.

Localization of Epileptic Focus Using Simultaneously Acquired EEGFMRI Data

Abstract
|
Affecting daily lives of millions of people, Epilepsy is a common central nervous system (neurological) disorder where cell activity in the brain is disturbed, causing recurrent seizures. Epilepsy can be treated commonly by medications. Be that as it may, medications do not always work as one may hope, and thus, some patients tend to resort to surgeries. The primary challenge in such surgeries, and by extension any other surgery where some part of brain may need to be disabled, disconnected or removed, is managing to pose no threat to the critical healthy textures adjacent or close to the part being operated on. Therefore, the precise localization of epileptic focus is a matter of vital importance in treating this condition. Various algorithms have been proposed to localize the brain sources and thus to determine the epileptic focus; however, none has yet been able to offer a solution to effectively address this issue. With EEG signal containing temporal information and fMRI carrying spatial information, it is hoped that the combination of the two can yield optimal results. In this research, we first remove the artifacts caused by EEG gradients, and proceed to study the signal in and outside the scanner by localizing the brain sources. The simultaneous processing of EEGfMRI enables us to make use of the temporal information in EEG to analyze fMRI. Epileptic foci are finally localized based on GLM method. This study has been conducted on 10 medicationresistant patients with epilepsy whose data was recorded in Iran National Brain Mapping Centre. The results suggest a significant improvement in localization accuracy compared to existing methods in the literature.
