پیش بینی کارکرد جنسی زناشویی براساس ابعاد ذهن آگاهی: نقش میانجی تصویر بدنی در زنان متاهل با جراحی ماستکتومی

مقدمه: مشکلات جنسی در بیماران مبتلا به سرطان پستان با پیشینه جراحی ماستکتومی موضوعی است که به شدت بر کیفیت زندگی آن ها تاثیر منفی دارد و با بالا رفتن هزینه درمانی و بهداشتی همراه است. لذا، پژوهش حاضر به منظور پیش بینی کارکرد جنسی زناشویی بر اساس ابعاد ذهن آگاهی (صفتی، ارتباطی و جنسی) و نقش میانجی تصویر بدنی در زنان مبتلا به سرطان پستان با پیشینه ماستکتومی انجام گرفت.روش بررسی: این پژوهش در قالب یک پژوهش توصیفی همبستگی و در سال 1402-1403 انجام گرفت. نمونه شامل 181 بیمار متاهل با جراحی ماستکتومی در استان کرمانشاه بود که به صورت در دسترس انتخاب شدند. برای بررسی اهداف پژوهش از پرسشنامه ذهن آگاهی جنسی  (smm)، ذهن اگاهی صفتی (ffmq)، ذهن آگاهی ارتباطی (rmm) و مقیاس کارکرد جنسی زناشویی (msfs) استفاده شد. نمونه گیری به دو صورت آنلاین و حضوری انجام گرفت. برای تحلیل داده ها از نرم افزار spss-25 و r استفاده شد. داده ها در بخش توصیفی با استفاده از میانگین، فراوانی، انحراف استاندارد و ضریب همبستگی پیرسون و در بخش آمار استنباطی با استفاده از روش مدل سازی معادلات ساختاری مورد تجزیه و تحلیل قرار گرفت.یافته ها: نتایج نشان داد که مدل پیشنهادی دارای برازش است. دو بعد ذهن آگاهی جنسی و ارتباطی به شکل مستقیم کارکرد جنسی زناشویی را پیش بینی کردند (0/01). همچنین، دو بعد ذهن آگاهی ارتباطی و جنسی با نقش میانجی تصویر بدنی به شکل معناداری توانستند کارکرد جنسی زناشویی را در زنان متاهل با پیشینه ماستکتومی پیش بینی کند (0/01).نتیجه گیری: درمان­گران برای بهبود کارکرد جنسی زناشویی مراجعین بهتر است مولفه ایی از جمله ذهن آگاهی جنسی، ذهن آگاهی ارتباطی و تصویر بدنی را هدف درمان قرار دهند. 

predicting marital sexual function based on the dimensions of mindfulness: the mediating role of body image in married women with a history of mastectomy

introduction: sexual problems in breast cancer patients with a history of mastectomy are an issue that impairs their quality of life and is associated with an increase in medical and health costs. the present study aimed to predict sexual function based on the dimensions of mindfulness (trait, relational, and sexual) and the mediating role of body image in women with breast cancer who have undergone mastectomy.materials and methods: this correlational descriptive research was conducted in 2023-2024. the sample included 181 married patients with mastectomy surgery who were selected using convenience method. sexual mindfulness measure (smm), five facet mindfulness questionnaire (ffmq), relationship mindfulness measure (rmm), and marital sexual function scale (msfs) were used to investigate the research objectives. sampling was conducted both online and in person. the spss (version 25) and r software were used for data analysis. the data were analyzed in the descriptive section using mean, frequency, standard deviation, and pearson correlation coefficient, and in the inferential statistics section using structural equation modeling.results: the results showed that the proposed model has a good fit. moreover, the two dimensions of sexual and relational mindfulness directly predicted marital sexual functioning (p<0.01). additionally, the two dimensions of relational and sexual mindfulness with the mediating role of body image could significantly predict sexual function in married women with a history of mastectomy (p<0.01). conclusion: to enhance the sexual function of patients, therapists are recommended to focus on components such as sexual mindfulness, relational mindfulness, and body image.
