بررسی تعیین حساسیت و اختصاصیت آنالیز فروزن سکشن مارژین‌ها طی جراحی حفظ پستان در بیماران مبتلا به سرطان پستان انوازیو و درجا و اهمیت آن در کاهش میزان جراحی مجدد

مقدمه: آنالیز فروزن سکشن حین عمل (iofsa) طی عمل جراحی حفظ پستان (bcs) می‌تواند نیاز به جراحی مجدد و پیامدهای آن را کاهش دهد. در مطالعه حاضر، حساسیت و اختصاصیت آنالیز فروزن سکشن برای تعیین بررسی مارژین‌ها در نمونه‌های پستان طی عمل جراحی حفظ پستان، در بیماران مبتلا به سرطان پستان انوازیو و درجا مورد ارزیابی قرار گرفت.روش بررسی: در این مطالعه بر روی 151 بیمار مبتلا به سرطان پستان، آنالیز فروزن سکشن حین عمل در حین جراحی حفظ پستان انجام شد، مارژین مثبت به‌صورت سلول‌های توموری (چه کارسینوم انوازیو و چه درجا) که تا حاشیه جوهر کشیده شده یا تا کمتر از 2 میلی‌متر از حاشیه جوهر فاصله داشت، تعریف شد. نتایج فروزن سکشن (وضعیت مارژین‌ها) به جراح گزارش شد. سپس بافت برای بررسی هیستولوژی قطعی و مقایسه بعدی فیکس شد و در پارافین قرار گرفت.یافته‌ها: نتایج به دست آمده نشان داد که حساسیت، اختصاصیت، ارزش اخباری مثبت، ارزش اخباری منفی و دقت آنالیز فروزن سکشن به ترتیب 42/ 96%، 73/ 94%، 52/ 91%، 82/ 97% و 36/ 95% درصد بود.نتیجه‌گیری:‌ نتایج ما نشان داد که استفاده از آنالیز فروزن سکشن در حین جراحی حفظ پستان می‌تواند منجر به کاهش پنج برابری نیاز به جراحی مجدد و خطر عوارض جراحی مجدد مانند عوارض بیهوشی، عوارض زیبایی، بار روانی و مالی شود.

evaluation of sensitivity and specificity of intraoperative frozen section analysis of margins in patients with breast carcinoma during breast-conserving surgery and its importance in reducing the rate of re-operation

introduction: intra-operative frozen section analysis (iofsa) during breast-conserving surgery (bcs) is recommended to reduce the need for reoperation and its complications. the present study aimed to assess the sensitivity and specificity of iofsa to determine the margin in specimens of breast cancer lumpectomy during bcs in patients with invasive and in situ breast carcinoma.methods: we performed iofsa during breast-conserving surgery for 151 patients with breast cancer and a positive margin was defined as tumor cells (either invasive or in situ carcinoma) extended to the inked margin or extended to within 2 mm of the inked margin. the results of frozen sections (margin status) were reported to the surgeon. then tissue was fixed and embedded in paraffin for definitive histological analysis and future comparison.results: the obtained results indicated that sensitivity, specificity, positive predictive value, negative predictive value, and accuracy of the frozen section were 96.42%, 94.73%, 91.52%, 97.82%, and 95.36%, respectively.conclusion: as evidenced by the results of this study, the use of iofsa during breast-conserving surgery could lead to a 5-fold reduction in the need for reoperation and risk of reoperation complications, such as anesthesia and cosmetic complications, as well as psychological and financial burdens.
