بررسی موانع انجام غربالگری ماموگرافیک در بستگان مبتلایان به سرطان پستان

مقدمه: سرطان پستان شایع ترین سرطان زنان در جهان و ایران و اولین علت مرگ و میر ناشی از سرطان در بین زنان ایرانی محسوب می شود. یکی از راه های کنترل این سرطان، انجام غربالگری ها و تشخیص زودهنگام این بیماری است. با توجه به این که امکان غربالگری در جمعیت عمومی وجود ندارد، تشخیص زودهنگام این سرطان در زنان پرخطر یکی از راه های کنترل این سرطان است. ماموگرافی یکی از راه های تشخیص این سرطان است، اما متاسفانه بیشتر افراد تمایلی به انجام ماموگرافی ندارند. این موضوع در زنان با سابقه فامیلی سرطان پستان، که یکی از گروه های پرخطر این بیماری محسوب می شوند، مساله مهمی است. به همین دلیل مطالعه حاضر با هدف شناسایی دیدگاه های بستگان بیمار مبتلا به سرطان پستان در زمینه عدم تمایل به انجام ماموگرافی انجام شده است.روش بررسی: مطالعه حاضر یک مطالعه کیفی بوده، که بر روی 160 نفر از بستگان درجه 1 و 118 نفر از بستگان درجه 2 بیماران مبتلا به سرطان پستان که به صورت نمونه گیری هدفمند انتخاب شده بودند، برای کشف دلایل عدم انجام ماموگرافی انجام گرفت. جهت جمع آوری داده ها، از مصاحبه های عمیق نیمه ساختارمند استفاده گردید. تمام داده ها ضبط و دست نویس شد و با استفاده از روش ساختاری مورد تجزیه و تحلیل قرار گرفت. یافته ها: در تحلیل محتوی مصاحبه ها، 5 مضمون، 14 طبقه اصلی، 73 طبقه فرعی استخراج شد، مسایل و مشکلات زندگی و عدم آگاهی کافی از دلایل عدم غربالگری در بستگان درجه 1 بوده و باورها و نگرش های نادرست در بستگان درجه 2 رتبه اول را بین دلایل ذکر شده آنان داشت. نتیجه گیری: از بستگان زنان مبتلا به سرطان پستان، با توجه به احتمال بالای ابتلا به این سرطان، غفلت می شود. پیشنهاد می شود که در سیستم خدمات بهداشتی کشور، آموزش های جامع و اثربخش برای بستگان زنان مبتلا به سرطان پستان با هدف ارتقاء آگاهی و رفع نگرش های نادرست، پیش بینی شود.

Barriers to Participation of Breast Cancer Patients’ Relatives in Mammographic Screening

Introduction: Breast cancer is the most common female cancer in the world and Iran and the leading cause of cancer death among Iranian women. One way to control this cancer is to get screened and diagnosed early. Given that screening in the general population is not possible, early detection of this cancer in highrisk women is one way to control it. Mammography is one way to diagnose breast cancer, but unfortunately, most people do not want to have a mammogram. This is an important issue for women with a family history of breast cancer, which constitutes one of the most highrisk groups for the disease. Therefore, the present study aimed to identify the views of relatives of patients with breast cancer about the reasons for unwillingness to perform mammography.Methods: The present study was a qualitative study performed on 160 firstdegree relatives and 118 seconddegree relatives of breast cancer patients who were selected by purposive sampling to discover the reasons for not performing mammography. Indepth semistructured interviews were used to collect data. All data were recorded and handwritten and analyzed using structural methods.Results: In the content analysis of the interviews, 5 themes, 14 main categories, and 73 subcategories were extracted. Preoccupation with daily life matters and lack of knowledge were the most common barriers to participation in screening among 1stdegree relatives, while wrong beliefs and attitudes constituted the most common barriers contributing to the unwillingness to getting mammography screening among 2nddegree relatives.Conclusion: Relatives of women with breast cancer are neglected despite facing a high probability of developing this cancer. It is suggested that the country’s health system provide comprehensive and effective education for relatives of women with breast cancer to raise awareness and eliminate misconceptions.
