بررسی تاثیر متوکلوپرامید بر کاهش رفلکس تهوع ناشی از قالبگیری

سابقه وهدف: یکی از نگرانی های دندان پزشکان بروز رفلکس تهوع بیماران هنگام قالب گیری است که منجر به محدودیت توانایی بیمار در پذیرش اعمال دندانپزشکی می شود. یکی از روش های دارویی استفاده از متوکلوپرامید است. هدف از انجام این تحقیق بررسی تاثیر متوکلوپرامید بر رفلکس تهوع هنگام قالبگیری بر روی بیماران بخش پروتزهای دندانی دانشکده دندانپزشکی دانشگاه آزاد اسلامی طی سال های 1397 و 1398 می باشد مواد و روش ها: تحقیق به روش کارآزمایی بالینی (از نوع مقایسه قبل و بعد) بر روی بیماران مراجعه کننده به بخش ثابت دانشکده دندانپزشکی انجام گرفت. بیماران باید از لحاظ پزشکی سالم بوده، هیچ گونه سابقۀ حساسیت دارویی یا آلرژی نداشته و هیچ یک از موارد منع مصرف و احتیاط مصرف متوکلوپرامید را نداشته باشند و هنگام قالبگیری حالت تهوع آنها براساس معیار vas بالای 5 باشد. شدت تهوع حین قالبگیری پیش از تجویز دارو ثبت شد. سپس یک میلی لیتر (25 قطره) حاوی 4 میلی گرم متوکلوپرامید به همراه یک میلی گرم سدیم ساخارین به عنوان شیرین کننده تجویز شد. بعد از نیم ساعت مجدداً شدت رفلکس تهوع حین قالبگیری ثبت گردید. میزان تغییر در شدت رفلکس تهوع با معیار vas با آزمون آماری mcnemar در محیط نرم افزاری spss v25 مورد بررسی قرار گرفت.یافته ها: تحقیق روی تعداد 28 نفر واجد شرایط، 46.4 درصد مرد و 53.7 درصد زن و در سنین 25 تا 56 سال که همگی رفلکس تهوع ناشی از قالبگیری رتبه بیشتر از 5 را داشتند انجام گرفت، بعد از استفاده از قطره متوکلوپرامید صد درصد بیماران شدت رفلکس تهوع 5 یا کمتر داشتند ( 0/0001˂p ). میزان رفلکس تهوع پیش و پس از تجویز متوکلوپرامید به ترتیب برابر با 3.3 ± 7.9 و 4.1 ± 1.3 بود. نتیجه‌گیری: بر اساس نتایج این مطالعه به نظر می رسد که تجویز متوکلوپرامید می تواند موجب کاهش رفلکس تهوع ناشی از قالب گیری در دندانپزشکی شود.

The Effect of Metoclopramide on Elimination of Nausea Reflex During Impression Making

AbstractBackground and aim: Nausea reflex during impressiontaking is a concern that decreases the patient's ability to accept dental procedures. There are several methods for elimination of nausea reflex, Prescription of Metoclopramide is one of these pharmaceutical methods. The aim of this study is to evaluate effect of Metoclopramide on the elimination of nausea reflex during impressiontaking in patients referring to Prosthodontics Department of the Faculty of Dentistry Tehran Medical Sciences, Islamic Azad University in 201819.Materials and Methods: This research is a clinical trial study (before and after study) which was performed on patients referring to fix Prosthodontics department, faculty of Dentistry. Patients should have no medical problem, without any history of drug or any other type of allergy. Only patients with a nausea reflex VAS benchmark over 5, were included in this study. The patients were asked not to eat since 3 hours before impressiontaking. Then the nausea reflex intensity before pharmaceutical treatment recorded. Afterwards, 1 ml (25 drops) of Metoclopramide hydrochloride along with 1 mg of sodium saccharin (as a sweetener) was prescribed. After half an hour, the intensity of the nausea reflex was measured again by the same VAS system, and the amount of change in the nausea reflex intensity was investigated by McNemar's statistical test in SPSS V25 software.Results: The study was performed on 28 eligible individuals, 46.4% male and 53.7% women, and 2556 years old, all of whom had a nausea reflex VAS of more than 5, while after using Metoclopramide, all patients (100%) reported nausea reflex score of 5 or less (P <0.0001). The reflex intensity before and after pharmaceutical treatment was 9/7 ± 3/3 and 3.1 ± 1.4, respectively.Conclusion: It seems that prescription of Metoclopramide can eliminate nausea reflex intensity during impressiontaking in dental patients.
