بررسی کیفیت زندگی مرتبط با سلامت دهان در بیماران مبتلا به مال اکلوژن

سابقه و هدف: مال اکلوژنتاثیر بسزایی بر رفتارهای اجتماعی و حالات روانی بیماران داشته و می تواند بر کیفیت زندگی مرتبط با سلامت دهان نیز تاثیرگذار باشد. هدف از این مطالعه بررسی کیفیت زندگی مرتبط با سلامت دهان در مراجعه کنندگان به بخش ارتودنسی دانشگاه آزاد اصفهان بود.مواد و روش ها: مطالعه توصیفی حاضر بر روی 77 نفر از متقاضیان درمان ارتودنسی که مبتلا به مال اکلوژن بودند به روش سرشماری انجام شد. قبل از ورود به درمان، افراد داوطلب پرسشنامه کیفیت زندگی مرتبط با سلامت دهان (ohip14) را تکمیل نمودند. سپس با استفاده از شاخص نیاز به درمان ارتودنسی (iotn) که شامل جزء زیبایی(ac) و جزء سلامت دندانی(dhc) می باشد، نیاز بیماران تعیین شد. جهت تجزیه و تحلیل داده ها از آزمون های t مستقل، من یو ویتنی، آنالیز واریانس یک طرفه، کروسکال والیس، ضریب همبستگی اسپیرمن و پیرسون و نیز رگرسیون خطی استفاده شد. یافته ها: میزان نمرهohip در بیماران (8/9 ±14/1) با دامنه 350 بود. در رابطه با جزء dhc، 65 درصد شرکت کنندگان در مطالعه نیاز زیاد یا خیلی زیاد به درمان داشتند. در رابطه با جزء ac، 33/83 درصد افراد نیاز قطعی به درمان ابراز کرده بودند. بین ohip و جزء (0/017p =، (0/28=r) و نیز بینohip و جزء (0/036p=، 0/24=r) ارتباط معنی دار اما ضعیفی وجود داشت. بر اساس رگرسیون خطی، ohip تنها با تحصیلات (0/026p =) و جزء ac (0/026p =) مرتبط بود.نتیجه گیری: اگرچه رابطه مال اکلوژن و کیفیت زندگی مرتبط با سلامت دهان در مطالعه حاضر ضعیف می باشد ولی توصیه می شود متخصصین ارتودنسی در کنار روش های حرفه ای برای تشخیص و طرح درمان، به تاثیر مشکلات دهان بر کیفیت زندگی بیماران نیز توجه داشته باشند.

Evaluation of oral health-related quality of life among patients with malocclusion

AbstractBackground and AIM: Malocclusion has an important effect on patients rsquo; social behaviors and mental situation and might affect their oral healthrelated quality of life (OHRQoL). The aim of this study was to assess OHRQoL in patients attending orthodontics department of Isfahan Azad University.Materials and Methods: The present descriptive study was conducted on 77 patients in need of orthodontic treatment applying a census sampling method. Before entering the treatment phase, volunteer patients filled out Oral Health Impact profile14 (OHIP14) questionnaire. Index of Orthodontics Treatment Need (IOTN): Aesthetic Component (AC) and Dental Health Component (DHC) were used to assess patient rsquo;s orthodontic treatment need. Student rsquo;s ttest, Mann U Whitney test, analysis of variance, kruskal wallis test, Pearson and spearman correlation coefficient, and linear regression model served for statistical analysis.Results: The mean score of OHIP was 14.1 ( ±8.9, range 035). Orthodontic treatment need was high or very high among 65% of the participants based on DHC. Based on AC, 33.8% of patients reported definite need to orthodontic treatment. A significant association existed between OHIP and DHC (r=0.28, P=0.017), and between OHIP and AC (r=0.24, P=0.036); however, it was weak. Based on linear regression model, OHIP was significantly associated with education (P=0.026) and AC of IOTN (P=0.026).Conclusion: Although, the relationship between malocclusion and OHRQoLin the present study was week, it is recommended for orthodontist to consider the effects of oral problems on patients rsquo; quality of life, in addition to their professional methods in diagnosis and treatment planning.
