استاندارد‌سازی فرایند گزارش‎ صبحگاهی گروه داخلی دانشگاه علوم پزشکی کاشان

مقدمه: گزارش صبحگاهی ازکاربردی ترین روش های آموزش بالینی است. این مطالعه با هدف بررسی وضعیت موجود گزارش صبحگاهی گروه داخلی دانشگاه علوم پزشکی کاشان به منظور شناسایی نقاط ضعف و آموزش استاندارد‌سازی این فرآیند مهم آموزشی انجام شد.روش‌ها: این مطالعه نیمه تجربی در گروه آموزشی داخلی دانشگاه علوم پزشکی کاشان در سال 94 انجام شد. ابزار جمع‌آوری اطلاعات پرسشنامه محقق‌ساخته بر اساس استانداردهای آموزش بالینی تدوین شده توسط حوزه ی معاونت آموزشی وزارت بهداشت، درمان و آموزش پزشکی بود. ابتدا با حضور در گزارش صبحگاهی و نظرسنجی از کارآموزان، کارورزان و دستیاران گروه نقاط ضعف شناسایی شد. نمونه گیری به صورت سرشماری از تمام فراگیران بخش داخلی در زمان مطالعه انجام شد (شامل 15 کارآموز، 6 کارورز و9 دستیار). سپس نحوه ی برگزاری صحیح گزارش صبحگاهی به اعضای هیات‌علمی با روش سخنرانی آموزش داده شد. پس از آموزش مجدداً با همان پرسشنامه نظرسنجی به عمل آمد و میزان تغییر در رعایت استانداردهای آموزشی قبل و بعد از تغییرات بررسی گردید. داده ها با استفاده آزمون‌های chis-quare test و fisher's exact test مورد تجزیه و تحلیل قرار گرفت. نتایج: استانداردهایی که افزایش رعایت کامل آن ها پس از مداخله معنا دار بود عبارتند از: رعایت کامل بحث مرحله به مرحله (0.001>p)، طرح تشخیص افتراقی (0.001=p)، بحث در مورد آزمایش های تشخیصی (0.001>p)، بحث در مورد رویکرد سیستماتیک (0.005=p)، بحث در مورد تصاویر رادیولوژی (0.002=p)، ایجاد موقعیت مناسب برای ارتباط استادان با فراگیران (0.004=p)، توجه به حضور و غیاب فراگیران (0.001=p)، ، ارتباط چشمی استادان با شرکت‌کنندگان (0.042=p).نتیجه‌گیری: آموزش اعضای هیات علمی گروه داخلی در زمینه ی گزارش صبحگاهی، در استاندارد نمودن آن تاثیر داشت و می توان با استمرار آن در جهت ارتقای کیفیت و انطباق بیش تر با استانداردها حرکت کرد.

Standardization of Morning Reports in Internal Medicine Department at Kashan University of Medical Sciences

Introduction: Morning report is one of the most effective methods in clinical education. The purpose of this paper was to assess the current state of morning report in the Department of Internal Medicine at Kashan University of Medical Sciences in order to identify the weaknesses and standardize this important educational procedure.Methods: This quasiexperimental study was conducted in Department of Internal Medicine at Kashan University of Medical Sciences in 2016. Data were collected by a researchermade questionnaire based on the clinical education standards developed by the Ministry of Health and Medical Education, Deputy of Education. First, the weaknesses were identified in the morning report sessions by surveying the stagers, interns and residents selected by means of census method from all of the internal medicine learners (15 stagers, 6 interns, and 9 residents). Then, faculty members were lectured on the correct way of holding morning report session. After the trainings, the participants were resurveyed using the same questionnaire and the extent of changes in complying with the educational standards was measured. The data were analyzed by Chisquare test and Fisher rsquo;s exact test.Results: The standards with significant increase in full compliance after the intervention were as follows: complete observation of ldquo;stepbystep discussion rdquo; (p<0.001), ldquo;differential diagnosis rdquo; (p=0.001), ldquo;discussion of diagnostic tests rdquo; (p<0.001), ldquo;discussion of systematic approach rdquo; (p=0.005), ldquo;discussion of radiology images rdquo; (p=0.002), ldquo;provision of appropriate conditions for teacherlearner interactions rdquo; (p=0.004), ldquo;attention to learners rsquo; attendance rdquo; (p=0.001), and ldquo;teacherlearner eyecontact rdquo; (p=0.024).Conclusion: The training of the internal medicine faculty members in morning report had a positive effect on the standardization, and the quality of education and its compliance with the standards may be improved by continuous trainings.
