عرصه‌های آموزش بالینی گروه‌های بزرگ از دیدگاه کارآموزان پزشکی عمومی دانشکده‌ی پزشکی اصفهان

مقدمه: برای هر دانشجوی پزشکی در دوره کارآموزی، آموزش بر بالین بیمار نقشی محوری ایفا می کند که می تواند میان دانسته های تئوری و مهارت بالینی وی پیوند بزند. هر سیستم آموزش کارآمد نیازمند ارزیابی مکرر است و آموزش پزشکی نیز از این قاعده مستثنی نیست. این مطالعه با هدف بررسی عرصه های آموزش بالینی گروه های بزرگ از نظر کارآموزان پزشکی عمومی دانشکده ی پزشکی اصفهان انجام شد. روش ها: در این مطالعه توصیفی مقطعی عرصه های آموزش بالینی (راند آموزشی و آموزش سرپایی) گروه های جراحی، داخلی، زنان و زایمان و اطفال بیمارستان های آموزشی وابسته به دانشگاه علوم پزشکی اصفهان از نظر کارآموزان در نیم سال نخست سال تحصیلی 95-94 مورد بررسی قرار گرفت. پژوهش در 61 گروه کارآموزی به صورت سرشماری شامل 21 گروه کارآموزی داخلی، 20 گروه کارآموزی جراحی، 7 گروه کارآموزی زنان و زایمان و 13 گروه کارآموزی اطفال در هفته نخست دوره انجام گرفت. برای جمع آوری اطلاعات از دفترچه پایش بالینی شامل تعداد دانشجویان شرکت کننده، ساعات حضور دانشجو، ساعات آموزش دانشجو توسط اساتید، تعداد بیمار ویزیت شده در هر روز، به مدت یک هفته به تفکیک عرصه های آموزشی استفاده شد. در انتها اطلاعات جمع آوری شده با نرم افزار excell به صورت میانگین و انحراف معیار محاسبه و گزارش شد.نتایج: کارآموزان به طور میانگین روزانه در عرصه های آموزش بالینی (راند آموزشی و آموزش سرپایی) داخلی 0/53±2/5 ساعت، جراحی 0/32 ±2/75 ساعت، زنان و زایمان 0/3±2/04 ساعت و اطفال 0/24±2/17ساعت در بخش ها و 0/25±2/75 ساعت در درمانگاه ها تحت آموزش اساتید قرار دارند. کارآموزان به طور میانگین روزانه در عرصه های آموزش بالینی بستری و سرپایی داخلی 1/55±5/54 بیمار، جراحی 1/25± 8/25 بیمار، زنان و زایمان 0/6±3 بیمار بستری و 5/02±22/2 بیمار سرپایی و اطفال تعداد 1/02±5/7 بیمار بستری و 5/14±15/9 بیمار سرپایی را ویزیت می کنند. نتیجه گیری: اطلاعات به دست آمده از میانگین تعداد کارآموزان، کارورزان، دستیاران، تعداد بیمار ویزیت شده در هر روز، تعداد ساعات حضور کارآموزان در بخش و آموزش توسط استاد می توان تصویری واقع بینانه از شرایط عرصه های آموزشی را به تصویر بکشدو به برنامه ریزان آموزش کارآموزان کمک نماید.

Major Clinical Training Fields from the Perspective of Medical Students at Isfahan University of Medical Sciences

Introduction: For any medical student, bedside training plays a pivotal role that could integrate his clinical skills and theoretical knowledge. Every efficient educational system requires regular evaluation and medical education is not an exception. The purpose of this study was to assess major clinical training fields from the perspective of medical students in Isfahan School of Medicine.Methods: This descriptive, crosssectional study assessed the clinical training fields (training round and outpatient training) of the surgery, internal medicine, gynecology, and pediatrics departments of teaching hospitals affiliated with Isfahan University of Medical Sciences from the perspective of medical students in the first semester of the academic year 201516. The study was conducted in the first week of the program on 61 groups of medical students selected by means of census method including 21 groups of internal medicine, 20 groups of surgery, 7 groups of gynecology, and 13 groups of pediatrics medical students. Data were collected by the notebook of clinical training monitoring to record the number of medical students attended, hours attended, hours of training by teachers, and the number of visited patients per day for a week categorized by the training fields. The data were analyzed in Excel using mean and standard deviation.Results: Daily training hours of the medical students (training rounds and outpatient training) were 2.5 ±0.53 (internal medicine), 2.75 ±0.32 (surgery), 2.04 ±0.3 (gynecology), and 2.17 ±0.24 (pediatrics) hours in the wards and 2.75 ±0.25 in the clinics. The medical students visited an average number of 5.54 ±1.55 inpatients and outpatients (internal medicine), 8.25 ±1.25 (surgery), 3 ±0.6 inpatients and 22.2 ±5.02 outpatients (gynecology), and 5.7 ±1.02 inpatients and 15.9 ±5.14 outpatients (pediatrics) per day.Conclusion: The information obtained in relation to the average number of medical students, interns and residents, the number of visited patients per day, and the hours of medical students rsquo; attendance and training by teachers in the wards can show a realistic picture of the conditions of training fields and help program planners.
