برآورد هزینه‌ها‌ی مستقیم و غیرمستقیم درمان یک‌ساله بیماران سرپایی پسوریازیس در ایران: مطالعه‌ای در بیمارستان رازی در سال‌های 99-1398

زمینه و هدف: بیماری پسوریازیس یا داءالصدف، بیماری پوستی مزمن خودایمنی التهابی سیستمیک و غیر قابل انتقال است که حدود 32 درصد مردم ایران مبتلا به این بیماری هستند. مدیریت هزینه‌ها و اختصاص بودجه و برنامه‌ریزی برای درمان بیماران مبتلا به پسوریازیس، نیاز به اطلاعات دقیق از هزینه‌های بیماران دارد. مطالعه حاضر به برآورد هزینه‌های مستقیم و غیرمستقیم درمان یک‌ساله بیماران در بین بیماران بیمارستان فوق‌تخصصی پوست رازی ایران می‌پردازد.روش اجرا: در این مطالعه توصیفی تحلیلی، هزینه‌های درمان 170 بیمار مبتلا به پسوریازیس، شامل هزینه‌های مستقیم (هزینه‌های پزشکی و غیرپزشکی) و هزینه‌های غیرمستقیم به روش سرشماری و با استفاده از چک‌لیست محقق‌ساخته از مهر ماه 1398 تا مهر 1399 محاسبه شد. هزینه ازطریق بررسی صورت‌حساب بیماران در سیستم اطلاعات بیمارستان و ازطریق مصاحبه تلفنی با بیماران اخذ شد.یافته‌ها: میانگین هزینه درمان یک‌ساله 9/037/480 تومان (758 دلار) که سهم هزینه مستقیم درمان برابر با 90.22 درصد (8/178/681 تومان) و سهم هزینه غیرمستقیم درمان برابر 9.78 درصد (858.799 تومان) بود. بیشترین هزینه درمان مربوط به خدمات دارویی با 5/825/233 تومان بود. همچنین کل هزینه‌‌‌‌ مستقیم پزشکی برابر با 7/069/890 تومان است که 62 درصد آن سهم بیمه و 38 درصد سهم بیمار بود. یافته‌ها نشان‌دهنده این بود که داشتن تزریق داروهای بیولوژیک و محل اقامت بر روی هزینه مستقیم درمان اثر معناداری دارد. هم‌چنین متغیر‌های بیمه تکمیلی و محل اقامت نیز بر هزینه‌های پرداخت از جیب تاثیر دارند.نتیجه‌گیری: بخش اعظم هزینه‌های درمان بیماری پسوریازیس را داروها تشکیل می‌دهند. قیمت بالای داروهای بیولوژیک و پوشش کمتر این داروها توسط شرکت‌های بیمه‌ای باعث مشکلات اقتصادی فراوانی برای این بیماران شده است. با توجه به یافته‌های این مطالعه، به‌نظر می‌رسد نیاز به بازبینی مجدد پوشش بیمه‌ای کامل‌تر این داروها و هم‌چنین استفاده از کمک‌های خیرین برای تامین بخشی از هزینه‌های درمان لازم باشد.

Estimation of direct and indirect costs of one year treatment for psoriasis outpatients in Iran: a study in Razi Hospital in 2017-2018

Background and Aim: Psoriasis is a systemic and nontransmissible chronic inflammatory autoimmune skin disease that about 23% of Iranians suffer from it. Cost of management and budgeting and planning for the treatment of patients with psoriasis requires accurate information. The present study estimates the direct and indirect costs for oneyear treatment of patients admitted in Razi Dermatology Hospital.Methods: In this descriptiveanalytical study, treatment costs of 170 patients with psoriasis, including direct (medical and nonmedical) and indirect costs were evaluated by census method and using a researchermade checklist, from October 2017 to October 2018. The fee was collected through reviewing patients #39; bills in the hospital information system and through telephone interviews with patients.Results: The average cost of one year was 9037480 Tomans ($ 758), of which direct cost of treatment was 90.22% (8178681 Tomans) and indirect cost of treatment was 9.78% (858799 Tomans). The highest cost of treatment was related to pharmaceutical services with 5825233 Tomans. Also, the total direct medical cost was equal to 7069890 Tomans, of which 62% was the insurance share and 38% was the patient share. The findings showed that injecting biological drugs and accommodation had a significant effect on the direct cost of treatment. Supplementary insurance and accommodation variables also affect outofpocket costs.Conclusion: Drugs made up most of the costs. The high price of biologics and the low coverage of these drugs by insurance companies have caused many economic problems for these patients. According to the findings of this study, it seems necessary to review the more complete insurance coverage of these drugs as well as the use of charitable assistance to cover part of the cost for treatment.
