عوامل موثر بر سطح عزت نفس در مبتلایان به آکنه: یک مطالعه مورد شاهدی

زمینه و هدف: آکنه یکی از شایع‌ترین اختلالات پوستی می‌باشد که بیش از 80 درصد افراد در سن بلوغ به آن مبتلا می‌شوند. این بیماری به دلیل تاثیر بر ظاهر بیماران می‌تواند آثار مخرب روانی همچون کاهش عزت نفس و درنتیجه انزوای اجتماعی ایجاد نماید. این مطالعه با هدف بررسی عوامل موثر بر سطح عزت نفس مبتلایان به آکنه در شهر همدان انجام شد. روش اجرا: بیماران دچار آکنه مراجعه‌کننده به درمانگاه پوست بیمارستان سینا همدان به‌عنوان گروه مورد و همراهان سالم آن‌ها به‌عنوان گروه شاهد وارد مطالعه شدند. پس از اخذ رضایت‌نامه کتبی اطلاعات دموگرافیک ثبت و پرسشگری از گروه‌ها انجام شد. برای تعیین شدت آکنه از global acne grading system (gags) و برای تعیین سطح عزت نفس از پرسش‌نامه کوپر اسمیت استفاده شد. در پایان داده‌ها با نسخه 14 نرم‌افزار stata آنالیز شدند. یافته‌ها: مجموعاً 184 بیمار و 177 نفر شاهد وارد مطالعه شدند. میانگین سنی بیماران و افراد شاهد به‌ترتیب 3.13 ±17.89 و 2.94 ±17.72 بود. براساس آزمون t مستقل، میانگین سطح عزت نفس در بیماران به‌صورت معنی‌داری کمتر از گروه شاهد بود (8.29 ±32.04 در مقابل 7.73 ±34.85، 0.001>p) شدت آکنه با میزان عزت نفس رابطه معکوس داشت و به‌ازای افزایش یک واحد امتیاز gags، 0.1 واحد میزان عزت نفس کاسته می‌شد (0.001>p). نتیجه‌گیری: ابتلا به آکنه بر افت عزت نفس افراد موثر است و شدت آکنه با میزان عزت نفس رابطه معکوس دارد.

Factors influencing the level of self-esteem in patients with acne: a case-control study

Background and Aim: Acne vulgaris is one of the most common skin disorders, affecting more than eighty percent of individuals around puberty. Because of the adverse effects on the patient rsquo;s appearance, the disease may have detrimental psychological impacts such as low selfesteem and social isolation. The aim of the present study was to evaluate factors affecting selfesteem in patients with acne vulgaris in Hamadan.Methods: Patients with acne vulgaris referred to the dermatology clinic of Hamadan rsquo;s Sina Hospital (as the case group), and their healthy accompanying persons (as the control group) were included in the study. After obtaining written informed consent, demographic data were recorded and participants were asked to fill out a questionnaire. To assess the severity of acne, Global Acne Grading System (GAGS) was used. In addition, Coopersmith Inventory was applied to measure the selfesteem of the participants. In data were analyzed using STATA 14.0 software.Results: Overall, 184 patients with acne and 177 controls were included in this study. The mean age of patients and controls was 17.89+3.13 and 17.72+2.94, respectively. Independent samples ttest showed that the mean SelfEsteem Index of patients was significantly lower than controls (32.04+8.29 vs. 34.85+7.73, P<0.001). The severity of acne was inversely correlated with selfesteem; one score increase in GAGS would result in 0.1 unit decrease in selfesteem index (P<0.001).Conclusion: Acne vulgaris is associated with low selfesteem and the severity of acne is inversely correlated with selfesteem level.
