مروری یکپارچه بر کاربرد مدل خودمراقبتی اورم در ایران

پیش زمینه و هدف: خودمراقبتی یکی از مفاهیم پایه و اساسی در مراقبت پرستاری بوده و برآیندهای مطلوبی را در تامین، حفظ و ارتقاء سلامتی افراد در پی دارد. مدل اورم یکی از مدل هایی است که بیمار را در مراقبت مشارکت می دهد و بر خودمراقبتی تاکید دارد. هدف از مطالعه حاضر بررسی مطالعات مختلف به صورت مرور یکپارچه و واکاوی کاربرد مدل خودمراقبتی اورم در ایران می باشد.مواد و روش ها: مطالعه حاضر از نوع مروری یکپارچه بود که در آن کلیدواژه های اورم، مدل اورم، خودمراقبتی، مدل/تئوری/چارچوب خودمراقبتی (و معادل انگلیسی آن ها) در پایگاه های داده ای elsevier، web of sciences، scopus، pubmed، proquest، cinahl، google scholar، ovid، sid، irandoc، iranmedex و magiran مورد جستجو گرفت. نتیجه جستجوی جامع اولیه شامل 28384 مقاله در کل پایگاه های داده ای بود که با اعمال فیلترینگ، معیارهای ورود/ خروج و مرور نقادانه با استفاده از چک لیست کانسورت، نهایتاً تعداد 24 مقاله انتخاب گردید.یافته ها: از میان 24 مطالعه موردبررسی، 7 مقاله در مجلات خارجی و 17 مقاله در مجلات داخلی به چاپ رسیده بودند. تمامی مقالات، مطالعات اصیل پژوهشی و مداخله ای بودند که ازنظر نوع، یک مورد تجربی، 11 مورد نیمه تجربی، 11 مورد کارآزمایی بالینی و یک مورد نیز کارآزمایی در عرصه بود. همچنین در تمامی مقالات، پژوهشگران از طرح پیش آزمون و پس آزمون بهره گرفته بودند. نتایج مستخرج از مقالات، در 3 حوزه اصلی شامل نوع مداخله انجام شده ، پیامد حاصل و جامعه موردمطالعه می باشد.بحث و نتیجه گیری: با توجه به نتایج به نظر می رسد که همچنان مشکلات بسیاری در مراقبت از بیمار و خودمراقبتی بعد از ترخیص وجود دارد، با توجه به دیدگاه مثبت و خوش بینانه مدل خودمراقبتی اورم نسبت به مشارکت بیماران در امر مراقبت خودشان که با ارزش های اجتماعی معاصر جامعه ایران هم راستا می باشد و با توجه به نقش پررنگ پرستار در ارائه مستقیم خدمات مراقبت سلامت در 2 سیستم جبرانی نسبی و کامل؛ به نظر می رسد این مفاهیم نیاز به بررسی دقیق تر و عمیق تر داشته تا مسئولین و متصدیان حوزه آموزش پرستاری با تهیه و تنظیم دستورالعمل های بالینی مدون و بومی و افزایش آگاهی پرستاران نسبت به مدل خودمراقبتی اورم، اقدام به ترجمان دانش نظری در عمل نمایند.

APPLICABLITY OF OREM’S SELF-CARE MODEL IN IRAN: AN INTEGRATED REVIEW

Background Aims: Selfcare is a basic concept in nursing care and has desirable outcomes in maintenance and promotion of human health. Orem's model focuses emphatically on selfcare and leads patients to participate and collaborate in their caring process. This study aimed to investigate several studies conducted on Orem's selfcare model through integrated review method and determine its applicablity in Iranian healthcare system.Material Methods: In an integrated review, all Iranian papers published during current 5 years (20112016) in both Persian and English language were searched using the related keywords of Orem's self care model in Elsevier, Web of Sciences, Scopus, PubMed, ProQuest, CINAHL, Google Scholar, Ovid, Magiran, SID, IranMedex, IranDoc databases. The quality of the articles was evaluated using CONSORT 2010 Statement. Finally, data were extracted, classified, summarized and, analyzed from 24 included articles in this study (Out of overall 28384 articles).Results: Out of 24 included articles, 17 artciles were published in Persian language and 7 articles were in English. All of the articles were original interventional studies (one experimental, 11 quasiexperimental, 11 clinical trial and one field trial). In all the papers, the researchers were used pretest and posttest design. Also, 3 main domains were derived from the literature review including: ldquo;type of intervention rdquo;, ldquo;intervention outcome rdquo; and ldquo;study population rdquo;.Conclusion: Given that still many problems remain in patient care and postdischarge selfcare and according to the positive and optimistic view of Orem rsquo;s selfcare model about patients rsquo; participation in selfcare and about nurses rsquo;s prominent role in healthcare provision in both partly and wholly compensatory systems which is consistent with the contemporary values of of Iranian society; it seems that the scrutiny of this model rsquo;s implications is needel. Evidently, the nursing officials and stakeholders would benefit from the preparation of local clinical guidelines and promotion of nurses rsquo; awareness about Orem rsquo;s selfcare model that leads to the ldquo;translation of theoretical knowledge in practice rdquo;.
