تاثیر بحث گروهی مراقبت محور بر روی فرسودگی مراقبین از بیماران تحت همودیالیز

پیش زمینه و هدف: با توجه به پیشرفت بیماری های مزمن ازجمله نارسایی مزمن کلیه و پیدایش مشکلات جسمی و روانی که برای آن ها در طی دوره ی بیماری و فرآیند درمان پیش می آید، نیاز به یک فرد برای مراقبت از این بیماران وجود دارد. فرد مراقب که معمولاً ً از اعضای خانواده بیمار می باشد در روند مراقبت ممکن است با مشکلات زیادی روبه رو شده که درنهایت موجب فرسودگی در آن ها می شود. این مطالعه باهدف تعیین تاثیر بحث گروهی مراقبت محور بر فرسودگی مراقبین از بیماران تحت همودیالیز طراحی و اجرا شد.مواد و روش کار: مطالعه ی حاضر یک کارآزمایی بالینی با روش نمونه گیری تصادفی بوده که بر روی 70 نفر از مراقبین بیماران تحت همودیالیز مراجعه کننده به مراکز آموزشی درمانی ارومیه در سال 1394 اجرا شد. ابزار گردآوری داده ها شامل پرسشنامه جمعیت شناختی و پرسشنامه فرسودگی مراقبین زاریت بود که در ابتدا به صورت پیش آزمون توسط نمونه ها تکمیل شد. سپس نمونه ها به طور تصادفی در دو گروه بحث گروهی و گروه کنترل قرار گرفتند. 2 هفته پس از اتمام برگزاری 6 جلسه بحث گروهی، پرسشنامه فرسودگی مراقبین زاریت مجدداً تکمیل و داده ها توسط نرم افزار آماری spss و به کمک آزمون های تی مستقل و من ویتنی مورد تجزیه و تحلیل قرار گرفتند.یافته ها: نتایج این مطالعه بیانگر آن بود بین دو گروه بحث گروهی و گروه کنترل ازنظر میانگین نمرات فرسودگی و ابعاد آن قبل از مداخله هیچ تفاوت آماری معنی داری وجود نداشت (05/0>p;)؛ اما پس از اجرای مداخله ازنظر میانگین نمرات فرسودگی و ابعاد آن تفاوت آماری معنی داری بین دو گروه مداخله و گروه کنترل مشاهده شد (05/0p<).بحث و نتیجه گیری: با توجه به نتایج مطالعه ی حاضر می توان چنین دریافت که برگزاری کلاس های آموزشی به روش بحث گروهی کاهش معنی داری در شدت فرسودگی مراقبین داشته است. لذا پیشنهاد می شود در برنامه ریزی های سیستم درمانی به نقش اعضای خانواده در درمان این بیماران توجه شده و ازاین روش آموزشی نیز بهره گیرند.

THE IMPACT OF CARE- CENTER GROUP DISCUSSION ON HEMODIALYSIS PATIENTS’ CAREGIVER BURNOUT

Background Aims: According to the development of chronic diseases, including chronic renal failure and the appearance of physical and mental problems that occurs to them during the period of disease and Treatment process, they require a person to take care of them. Caregiver that he/she is a family member of a patient in the care process may be faced with many problems that eventually led to the emergence of burnout in it gets. The aim of this study is to determine the effect of care centered group discussion on the caregiver burden of patients undergoing hemodialysis.Materials Methods: This study was a randomized clinical trial. Participant of this study contain 70 caregivers of patients undergoing hemodialysis in educational therapeutic centers in Urmia. Caregivers selected and allocated randomly in two groups of intervention (group discussion) and control. Demographic and caregiver burden questionnaire completed by caregiver before the intervention. This program consists of 6 sessions of group discussion during 6 weeks. Two weeks later of intervention the caregiver burden of questionnaire will be completed by caregiver in intervention and control groups. At next the data obtained from questionnaire analysis of descriptive and analytic statistics.Results: Results showed that there was no significant difference between the study and control groups before intervention in the mean scores of burnout and its dimensions (p>0.05.). But after the intervention there was significant difference in the mean scores of burnout and its dimensions between intervention and control groups (p<0.05.).Conclusion: The results of this study can be found that training classes such as group discussion had a significant reduction in the severity of caregiver burnout. Therefore it is recommended that in the Health system planning attend to the role of family members in the treatment of these patients also benefit from this training method.
