مرور تاثیرات لپتین بر بارداری و پیامد نوزادی: مطالعه مروری

پیش زمینه و هدف: لپتین سرم، هورمون در گردشی است که به میزان زیادی در بافت چربی تولید میشود. نقص در ژن لپتین و رسپتور آن باعث مشکلات پاتولوژیک عدیدهای ازجمله پرخوری و چاقی، عملکرد نادرست تولیدمثل و عدم تعادل هورمونی میشود. مقصود از این بازنگری، خلاصه کردن و تحلیل مقالات انجامشده در ارتباط با تاثیرات لپتین بر بارداری و پیامد نوزادی است.مواد و روشها: در این مطالعه مروری ساده، جستجو در بانکهای اطلاعاتی pubmed، medline، irandoc،sid، magiran و science direct در ارتباط با تاثیرات لپتین بر بارداری و پیامد نوزادی انجام شد. درمجموع 120 مقاله موردبررسی اولیه (بررسی چکیده) قرار گرفت و 70 مقاله ازنظر مرتبط بودن با عنوان مقاله و دارا بودن شرایط ورود، انتخاب شدند و موردبررسی ثانویه (بررسی مقاله) قرار گرفتند.یافته ها: ارتباط معنی داری بین وزن مادر و سطح سرمی لپتین او یافت شد. بررسی ها نشان داد که در عوارض بارداری میزان لپتین افزایش مییابد. سطح لپتین بند ناف با وزن هنگام تولد، قد و دور سر نوزاد ارتباط معنیداری داشت. به علاوه، تجمع این پلی پپتید در عروق بند ناف با وزن جفت ارتباط معنیداری داشت. در شیر مادر نیز این هورمون یافت میشود که میتواند سبب جلوگیری از چاقی در نوزاد گردد.بحث و نتیجه گیری: هورمون لپتین در فیزیولوژی، پاتوفیزیولوژی بارداری و پیامدهای نوزادی نقش دارد و شاید بتوان از این هورمون در پیشگویی پاتوفیزیولوژی بارداری و درمان بارداریهای همراه با محدودیت رشد جنین سود برد. اما در کل برای شناختن اثرات بیشتر این هورمون نیاز به مطالعات بیشتر میباشد.

THE EFFECTS OF LEPTIN ON PREGNANCY AND NEONATAL OUTCOME, A REVIEW

Received: 24 Jun, 2015 Accepted: 1 Sep, 2015AbstractBackground Aims: Serum leptin is a circulating hormone that is expressed abundantly in adipose tissue. Defects in the leptin gene and its receptor causes numerous pathological problems such as over eating and obesity, improper functioning of the reproductive and hormonal imbalances. The purpose of this review is summarizing and analyzing articles about the effects of; leptin on pregnancy and neonatal outcome.Materials Methods: The Pubmed, Medline, Irandoc, SID, Science direct, Magiran database were surveyed for original studies describing the association between leptin effects on pregnancy and neonatal outcome. Totally, 120 articles were reviewed and 70 articles of association; with leptin effects on pregnancy and neonatal outcome were selected and investigated secondarily (review essay).Results: A significant relationship was found between maternal weight and maternal serum leptin. Studies have indicated that leptin levels are increased in pregnancy complications such as preeclampsia and diabetes. Cord leptin level was significantly associated with birth weight, length and head circumference of infant. In addition concentrations of the polypeptide in umbilical cord blood were correlated with placental weights. This hormone is also found in breast milk that can prevent obesity in the infant.Conclusions: Many physiological roles have been suggested for leptin in human pregnancy. Leptin also plays a role in the pathophysiology of pregnancy, and it may be predictive in the complication of pregnancy. Studies have suggested that recombinant human leptin can be used; in the treatment of pregnancies with fetal growth restriction.
