اخبار بد شایع در بخش‌های مراقبت ویژه و اورژانس و میزان مشارکت پرستاران در فرایند انتقال خبر بد در بیمارستان‌های وابسته به دانشگاه علوم پزشکی تهران

پیش‌زمینه و هدف: انتقال خبر بد یکی از چالش‌های موجود در بخش‌هایمراقبت ویژه و اورژانس است. پرستاران به‌عنوان کسانی که وظیفه مراقبت مداوم از بیماررا به عهده دارند نقشی مهمی در فرایند انتقال خبر بد دارند، درحالی‌که کمتر بهبررسی ابعاد مشارکت آن‌ها در این زمینه پرداخته شده است. این مطالعه به‌منظور بررسینوع اخبار بد شایع در بخش‌های ویژه و اورژانس و تعیین میزان مشارکت پرستاران درفرایند انتقال خبر بد انجام شد. مواد و روش‌ها: در این مطالعه توصیفی،182 نفر از پرستاران بخش‌های ویژهو اورژانس بیمارستان‌های دانشگاه علوم پزشکی تهران به روش نمونه‌گیری طبقه‌ای و سهمیه‌ای موردبررسی قرار گرفتند. ابزارگردآوری داده‌ها، پرسشنامه بود که شایع‌ترین اخبار بدی که پرستاران با آن مواجههداشتند و نوع فعالیت‌هایی که در زمینه انتقال خبر بد انجام می‌دادند را به طریق خودگزارش‌دهیمورد بررسیقرار می‌داد. یافته‌ها: بر اساس یافته‌ها، اخبار مربوط به مرگ بیمار بیشترین اخباری هستند کهپرستاران در بخش‌های ccu (89.5درصد)، icu (77.3درصد) و اورژانس (81.8درصد) با آن مواجهه دارند. مهم‌ترین فعالیت‌هایمرتبط با انتقال خبر بد که پرستاران اغلب در آن مشارکت داشتند عبارت بودند از حمایتعاطفی از بیمار و خانواده وی (0.95±2.79)، توضیح صحبت‌های پزشک با بیان ساده‌تر (0.96±2.68)، کمک به بیمار و خانوادهبرای سازش با خبر بد (0.89±2.63)، آماده کردن بیمار یا خانواده برای شنیدن خبربد (1.04±2.6).همچنین مشخص شد 34.1درصد پرستاران، اغلب خبر بد را مستقلاً به بیمار یا بستگان وی منتقل می‌کنند. بحث و نتیجه‌گیری: این پژوهش نشان داد گستره اخبار بد در بخش‌های ویژهو اورژانس متنوع بوده و پرستاران اغلب در فعالیت‌های مرتبط با فرایند انتقال خبربد مشارکت دارند، لذا آموزش آن‌ها درباره چگونگی انتقال خبر بد و تدارک سازوکارهایحمایتی از ایشان در این زمینه ضروری به نظر می‌رسد.

BAD NEWS IN CRITICAL CARE AND EMEREGENCY UNITS AND NURSES’PARTICIPATION IN THE PROCESS OF BREAKING BAD NEWS IN AFFILIATED HOSPITALS OF TEHRAN UNIVERSITY OF MEDICAL SCIENCES

Bad news in criticalcare and emeregency units and nurses’participation in the process of breakingbad news in affiliated hospitals of Tehran University of Medical Sciences  Received: 4 Feb, 2015 Accepted:10 May, 2015 Abstract: Background Aim: Breakingbad news is one of the challenges in intensive care units and emergencydepartments. Nurses as a person who has responsibility for continuous caring,has a significant role in the process of breaking bad news, but little has been done to explore this issue.The purpose of this study was to determine types of bad news common inintensive care units and emergency departments and to investigate nurses‘participation in the process of breaking bad news. Materials Methods: Inthis descriptive study, a sample of 182 nurses working at intensive care unitsand emergency departments of thehospitals affiliated to Tehran University of Medical Sciences selected in a quota and stratified samplingmethod. The data collection tool was a selfreported questionnaire which query common bad newsthat nurses encounter, and the activities they involved in breaking bad news. Results:According to the findings, news about death was the most common news in CCU(89/5%), ICU (77/3%), and emergency departments (81/8%). The most importantactivities that nurses had been involved were emotional support of patients andrelatives (2/79±0/95), explaining the physician’s speech in a simple manner (2/68±0/96), helping patients orrelatives adapt with bad news over time (2/63±0/89), preparing the patient or his/her relative for hearing bad news(2/6±1/04). Theresults showed 34.1% of nurses break bad news to a patient or his/her relative independently. Conclusion: Thisstudy showed the scope of bad news in intensive care units and emergencydepartments is broad and diverse. Also, nurses are mostly involved in activities related to breaking badnews.
