بررسی رابطه بین بصیرت به بیماری با میزان پذیرش درمان در بیماران مبتلا به اسکیزوفرنی

پیش زمینه و هدف: عدم تبعیت از دارودرمانی به عنوان یک مشکل چالش انگیز در مدیریت بیماران مبتلا به اسکیزوفرن می باشد که منجر به پیامدهای ضعیف بالینی و عملکردی در این بیماران می شود. عوامل مختلفی در پذیرش دارودرمانی بیماران دخیل هستند که بصیرت نسبت به بیماری می تواند یکی از این عوامل باشد. این مطالعه با هدف تعیین ارتباط میان بصیرت نسبت به بیماری و پذیرش دارودرمانی در بیماران اسکیزوفرن انجام شد. مواد و روش ها: در این مطالعه توصیفی تحلیلی، 130 بیمار مبتلا به اسکیزوفرنی بستری در مرکز آموزشی درمانی رازی تبریز انتخاب شد با رعایت معیارهای ورود و با در نظر گرفتن 10درصد ریزش نمونه به علت عدم وجود معیارهای ورود، حجم نمونهٔ نهایی، 80 نفر انتخاب گردید. جهت گردآوری داده ها از پرسشنامه الگوی باور بهداشتی جهت بررسی بصیرت نسبت به بیماری و فهرست وارسی جهت بررسی پذیرش دارودرمانی استفاده شد. بعد از تصویب طرح و رعایت پروتکل های اخلاق در پژوهش داده ها به وسیله مصاحبه و مشاهده جمع آوری شدند و با استفاده از نرم افزار spss و آمار توصیفی (فراوانی، درصد، میانگین و انحراف معیار) و استنباطی (آزمون ضریب همبستگی پیرسون) تجزیه وتحلیل شدند. یافته ها: اکثر شرکت کنندگان زن، مجرد، بیکار و دارای تحصیلات زیر دیپلم بودند. یافته های این مطالعه نشان می دهد که اکثریت بیماران (62درصد) پیروی خوبی از رژیم دارویی داشتند. نتایج آزمون همبستگی پیرسون نشان داد که میزان پیروی از رژیم دارویی و درک فردی از بیماری (p=0/02) و پیروی از رژیم دارویی و استعداد فردی نسبت به بیماری (p=0/001) ازلحاظ آماری ارتباط معنی دار دارد. بحث و نتیجه گیری: بصیرت نسبت به بیماری در بیماران مبتلا به اسکیزوفرنیا با پذیرش دارودرمانی ارتباط دارد. با توجه به کمبود مطالعات انجام شده در این زمینه نیاز به مطالعات بیشتر جهت قوت بیشتر داده ها ضروری به نظر می رسد.

Relationship between INSIGHTS into the DISEASE WITH compliance in schizophrenic patients Ebrahimi H[1], Namdar H[2], ranjbar F[3], Mosaffa F[4] * Received: 26 Apr , 2014 Accepted: 23 June , 2014 Abstract Background Aims: Noncompliance is a challenging problem in the management of schizophrenic patients that results in poor clinical and functional outcomes in these patients. Different factors are effective in patients compliance, insight into the disease could be one of them. This study tries to determine the relationship between patientrsquo s insight into the disease and the compliance in schizophrenic patients. Materials Methods: In this descriptive analytical study 80 hospitalized schizophrenic patients from Razi educational clinical center in Tabriz were chosen by considering the purposive sling method criteria. In order to collect the data, we used health belief model questionnaire to review insight into the disease, and a check list to review the compliance. After taking into account the research ethic protocols, we collected data by interviewing and observation. The data were analyzed by using SPSS software, descriptive statistics (abundance, percentage, mean, and standard deviation) and interpretive statistics (Pierson correlation test). Results: Most participants were female, single, unemployed with education less than high school diploma. According to the results of this study most patients had good compliance (62%). According to Pierson correlation test, there is a meaningful statistical relationship between patientrsquo s compliance and patientrsquo s insight into the disease (p=0.02) and also between compliance and patientrsquo s susceptibility to disease (p=0.001). Conclusion: There is a relationship between patientrsquo s insight into the disease, and compliance in schizophrenic patients. Due to the lack of vast studies in this subject, more studies are necessary to be done.
